We deliver exceptional, personalized care that improves health, lowers costs, and empowers people to live their best lives. With deep roots in clinical assessment and care management services, our national network of clinicians breaks through traditional barriers to care by meeting those members wherever they call home. We help older adults and other at-risk individuals enjoy a better quality of care, experience improved health outcomes, and identify chronic conditions that may otherwise go undiagnosed.
Our job opportunities allow you to leverage your expertise and compassion to make a direct impact on the health and well-being of others. Join our team and be rewarded with competitive compensation and flexible scheduling while making a difference in the community!

Custom developed technology driven solutions for psychiatry including an integrated EMR which supports provider-to-patient messaging, telemedicine, direct lab integration and proprietary tools to track and gain insight into your patients' outcomes.
Spend time with your patients. Minimum of 60 minutes for intakes and 30 minutes on follow up visits. Our model is built upon providing supportive therapy, in addition to medication management, to our patients.
Strong in-house referral network that allows you to refer patients to specialists and therapists whose practice supports the patients you are actively treating. Best of all, clinical information is shared seamlessly through our EMR.
