As an Infection Preventionist, you will play a vital role in developing, implementing, and monitoring programs designed to prevent the spread of infections and communicable diseases. You'll work closely with facility leadership, clinical teams, and quality committees to ensure compliance with state and federal infection control standards and promote best practices across the organization. Your expertise will help shape policies, provide ongoing staff education, and contribute to a culture of safety and continuous improvement throughout our center.
RESPONSIBILITIES:
• Develop, implement, and oversee the facility's infection prevention and control program in accordance with applicable federal, state, and local regulations.
• Conduct ongoing surveillance, track, and trend infections and healthcare-associated infection data and report findings to leadership and regulatory agencies.
• Review, update, and direct the implementation of infection control policies and procedures based on current evidence and guidelines.
• Provide education, training, and in-service programs on infection prevention practices for staff, new hires, and relevant stakeholders.
• Serve as a subject matter expert and resource for facility staff, residents, and families on infection prevention practices and compliance.
• Participate in interdisciplinary committees such as Quality Assurance & Performance Improvement (QAPI) to integrate infection control priorities.
• Conduct rounds and audits to assess adherence to infection prevention protocols and identify areas for improvement.
• Coordinate outbreak investigation and response efforts, including initiating control measures and communication with public health authorities.
• Ensure compliance with employee health requirements, immunization programs, and screening documentation per regulatory standards.
• Prepare and present regular reports on infection trends, compliance, and improvement actions to facility leadership.
• Collaborate with clinical leadership, department heads, and external partners (such as public health agencies) on infection prevention initiatives.
• Support process improvement activities using infection data and surveillance results to reduce infection risks.
QUALIFICATIONS:
• Valid, unrestricted RN license in Connecticut.
• Degree in nursing, public health, microbiology, epidemiology, medical technology, or a related healthcare field (BSN preferred).
• Certification in Infection Prevention and Control (e.g., CIC or LTC-CIP) - or willingness to obtain within the first year of employment per facility and regulatory expectations.
• Current CPR/BLS (or equivalent) certification as required by facility policy.
• Minimum clinical experience, ideally 2-3 years of healthcare experience, with preference for background in infection prevention, long-term care, or related clinical work.
• Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.
• Effective organizational, analytical, and time-management skills, with the ability to track data, identify trends, and help drive infection prevention initiatives
• Compassionate, resident-centered approach, with good organizational and time-management skills.
• Comfortable working in a long-term care or skilled nursing environment (experience preferred).
• Be able to read, write and understand English.
• Ability to work independently and serve as a resource and leader in infection prevention practices throughout the facility.

Wound Care: NPs/PAs are skilled in wound assessment and treatment, promoting healing and preventing complications.
Fall Prevention: They identify residents at risk for falls and implement strategies to reduce fall incidents.
Knowledge of CMS Regulations and NCQA HEDIS Guidelines
Document accurate medical records to ensure compliance with OSHA and
Government regulations and perform duties in accordance with site quality and
safety systems
Collaborate with the interdisciplinary team
Analyze and organize the care of critically complex patients
Requirements
Minimum 6 months of experience in long-term care or a skilled nursing facility
Active and unrestricted NursePractitioner, Physician Assistant, or Physician license
DEA License
CPR BCLS/BLS certified
Must hold a Master's Degree in Nursing
Completed a degree from an accredited NursePractitioner program
Preferred experience in cardiology: training provided
Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
Must have great organizational and time management skills.
Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day

We are seeking a Pediatric NursePractitioner (PNP, APRN), with post-graduate clinical experience, preferably in a primary care setting to join our Department of Pediatrics at Community Health Services (CHS). The PNP will deliver comprehensive quality patient care, including diagnostic and therapeutic interventions according to standards of practice to children and adolescents. The Clinical Director of the department is Dr. Zeev Harel, former Brown University Medical School Professor of Pediatrics. Levels of employees' engagement and satisfaction in this department are above national average. Although clinical experience is preferable, recent graduates are also encouraged to apply.
The PNP will provide health care in collaboration with department physicians. He/she may refer patients to consultants for confirmation of diagnosis and treatment procedures as appropriate. The PNP will also participate in departmental quality improvement and peer review activities.

The successful candidate will:
Be a graduate of an accredited school of nursing and have successfully completed an approved nursepractitioner program, specializing in pediatric medicine.
Possess valid State of Connecticut licensure.
Have or be eligible to obtain DEA and state of Connecticut controlled substance registration.
Have a deep knowledge of nursing theory and practice coupled with a record of providing high quality, comprehensive, professional clinical care to patients according to established standards, protocols and practices.
Possess the ability to provide cultural competent care to a diverse patient population in a collaborative environment.
Have skill taking medical history, assessing medical conditions, and interpreting findings.
Have a track record of proven success communicating effectively with patients, peers, and other stakeholders both orally and in writing.
Have demonstrated ability to maintain timely, thorough and accurate written clinical documentation.
Be able to make evidence-based clinical decisions and comply with department and organizational quality measures.
Be comfortable using electronic health record and other computer systems.
Coordinate patient care with team members, other departments, and community agencies/providers as necessary.
Be willing to perform other duties as assigned.
