Locum Tenens Family Nurse Practitioner Home Health Assessments ??? Near Port Reading, NJ (Ocean County) We are seeking dedicated and dynamic Nurse Practitioners specializing in Family Practice or Acute Care for a 90-day locum tenens assignment. This role involves in-home health assessments throughout Ocean County, NJ, starting June ongoing. Key Responsibilities: Perform 5???6 home health assessments per day. Conduct annual risk assessments and review chronic conditions with patients. No treatment or prescribing involved; this is an assessment-only role. Escalate any urgent findings to the patient???s primary care physician. Conduct A1C tests if applicable; other screenings will be conducted by the patient. Travel is required, and mileage will be reimbursed daily. Schedule: Full-time, 40 hours per week, Monday through Saturday. Scheduled 8-hour shifts within the 8 AM to 8 PM timeframe, with occasional flexibility for a 12 PM to 8 PM schedule. Includes a one-hour lunch break. Schedules released 2???3 weeks in advance. Requirements: Board-Certified Family Nurse Practitioner or Acute Care Nurse Practitioner. Must hold an active New Jersey license and Medicare number.
