The Opportunity:

Full time 40 hours including evenings and weekends
What you'll be working on:
Treating patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups
Continuous learning during weekly Clinical Rounds and through other modalities
Ongoing collaboration with both virtual and in-office teammates via daily huddles
Utilization of your specific clinical training and opportunities to give exceptional care to patients virtually
Education, licenses, and experiences required for this role:
Completed an accredited FNP program with a national certification
In the past 5 years, practiced as an Advanced Practitioner for at least:
2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
Currently licensed in CA with ability to obtain additional state licenses
Ability to work weekday and weekend shifts (every other Saturday AND or Sunday required)
Current shifts range from (6am - 12am PST)

The Registered Nurse - Clinic/Wellness/Immunization works in a variety of either remote or clinic locations to serve a specific population's health and wellness needs. The Registered Nurse demonstrates the ability to make clinical judgments effectively and efficiently to provide quality appropriate care in accordance with facility's policies and protocols. These variety of sites include but are not limited to community or employment immunization clinics, wellness clinics, job safety and workers compensation sites and travel health clinics from pediatric to geriatric age populations.
Essential Duties and Responsibilities:
Evaluates a patient's health status, estimates the level of health risk and informs/provides feedback to patients to motivate behavior change to reduce health risks
Identifies and prioritizes actions based on patient care requirements
Performs interventions according to identified priorities, plan of care, and clinic/site policies
Provides a safe, comfortable and therapeutic patient environment
Initiates patient and family education when appropriate
Documents patient's responses to nursing care and other interventions as well as follow up recommendations
Monitors the safety and effectiveness of the environment and equipment
Must have working knowledge of universal precautions
Knowledge of protocols for reporting adverse reactions, incidents, and injuries
Knowledge of Workers Compensation assessments and reporting if applicable
Knowledge of Immunization and Vaccine schedules and safety
Administers vaccines or any necessary medications per site/facility protocols
Appropriately stores, transports, and handles vaccines
Safely and appropriately transports biological material, sharps, and other required supplies to clinic sites
Appropriately handles and disposes of biological waste
Completes appropriate participant consent forms and clinic records
Inputs information into the client's database for tracking per policy/protocol and state regulations if applicable
Minimum Requirements:
Current Registered Nurse License for the state in which the nurse practices
One year experience as a nurse in a clinic or immunization setting preferred • Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age

How You Will Contribute:
The Psychiatric Mental Health NursePractitioner (PMHNP) is a key member of our multidisciplinary treatment team. In this role you will be responsible for conducting psychiatric evaluations, medication consultation, and psychiatric follow up appointments for our clients. In addition to Psychiatric duties, the PMHNP also completes nursing duties to assess and evaluate the medical functioning and stability of patients within the program.

Essential Responsibilities:
Completes Psychiatric Evaluations and ensures medical necessity for all individuals enrolled in the PHP and IOP programs.
Provides medication management services for individuals enrolled in PHP as needed. Available on-site as scheduled and by telephone in unusual or urgent situations related to medication.
Provides consultation, education and related information to patients and the families to increase success of treatment and program engagement.
Collaborates as part of a multi-disciplinary team with staff on clinical treatment planning and recommendations.
Completes medical record and other documentation in a timely and accurate manner as needed to meet clinical, payer and other requirements.
Serves as first point of contact in addressing, supporting, and triaging patient's physical complaints and discomfort.
As part of the admission process and ongoing treatment, conducts patient medical assessment; (1) performs phlebotomy to collect blood for lab analysis, (2) completes comprehensive nursing assessment, (3) completes Electrocardiogram (EKG), (4) collection of basic vitals including height, weight, temperature, and blood pressure, (5) basic urinalysis testing.

What We Are Seeking:

Perform comprehensive psychiatric evaluations that include evaluation of mental status, current psychiatric symptoms and functioning, and other relevant health histories.

Role description
Under general nursing and medical direction, utilizing standardized procedures, the NursePractitioner functions within an organized system for the delivery of health care in association with other members of the multidisciplinary health care team and performs specialized nursepractitioner duties in clinical settings.

* Minimum of six (6) months (equivalent to 1,000 hours) of Occupational Health experience.
* One (1) year (equivalent to 2,000 hours) of experience as a Registered Nurse on a psychiatric inpatient unit, psychiatric emergency service clinic or mental health center.
* One (1) year (equivalent to 2,000 hours) of experience in a setting that treats HIV, AIDS, and Sexually Transmitted Infections.
One-year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

Eligible List/Score Report:
