Palliative Care Certification preferred. Current Basic Life Support license (BLS). Experience: Minimum of year (1) year experience in Hospice, home health, palliative, or similar is preferred. Core Competencies: Possess critical thinking skills, in-depth knowledge of clinical disease entities common to Hospice, Home Health, transitional care management and palliative care including the pathophysiology, disease progression, pharmacological and non-pharmacological treatment modalities for palliative care, in-depth knowledge of the philosophy, the Medicare benefit, and Conditions of Participation. Skills: Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist. Other: Valid driver's license and auto insurance. FUNCTIONS AND RESPONSIBILITIES: 1. Developing and assuring, in collaboration with the staff, implementation of policies for the medical care of patients. 2. Providing counsel regarding medical issues within the team and relationships with the medical community. 3. Advising and reporting activities and developments that relate to the medical care, of patients. 4. Promoting the program, including developing and maintaining effective and positive relationships with health care professionals within the community. 5. Participating in the continuing education of the Interdisciplinary Team Conferences to review patient care planning. 6. Communicating with Nurse Practitioner's or other health practitioners providing services for patients at appropriate intervals, serving as a resource person in medical management. 7. Participating in routine record review of patients. 8. Providing orientation/educational information to such team. 9. Presenting in-service programs and makes educational presentations to staff and volunteers, nursing home staff, hospital staff, medical students, nurses and resident physician's assistants to promote understanding of program concepts, medical techniques, problem management and promotion of good working relationships. 10. Remaining current in the techniques of palliative medicine, through continuing medical education, medical journals, clinical research, and peer contact. 11. Participating in utilization review and quality assurance activities, including chart review and audits, and resolving physician's assistant conflicts. 12. Selecting and supervising associate practitioners as approved by the corporate office. 13. Acting in accordance with company policies and procedures. 14. Participating as a member of the interdisciplinary group that establishes and/or updates the individual's plan of care, when serving as the attending physician; if Nurse Practitioner may not serve as or replace the medical director or physician designee. 15.
