Position Summary
Advance Community Health is actively looking for a PsychiatricNursePractitioner who is passionate about patient care and committed to clinical excellence. The PMHNP is responsible for providing psychotherapy, treatment planning behavioral evaluation, medication evaluation and management to a range of clients having mental illness, mental retardation, and substance abuse disorders under the supervision of a consulting psychiatrist(s).

Essential Duties and Responsibilities
Conducts therapy, medication assessments and crisis response to new and existing clients.
Develops individual service plans based upon assessment data and provides therapeutic interventions as necessary including the provision of on-going medication management services.
Provides consultation and health education to agency staff, caregivers, and clients as it is relative to the treatment and medication needs of the patient.
Maintains documentation of services including service plans, progress notes, medication informed consent, quarterly treatment plan reviews and other required information in compliance with organization and third-party compliance standards.
Receives medical supervision from psychiatrist consistent with applicable PPFNP (Protocol for Provision of Family NursePractitioner Services)
Involves social work and ancillary staff appropriately.
Appropriately and timely communicates issues to supervising physician.
Appropriately communicates problems to Administration.
Participates in regularly scheduled provider meetings and committees.
Maintains CME/credentialing/licensure in compliance with state and board requirements.
Maintains patient confidentiality. Responds appropriately to patient satisfaction concerns.

FNP/PMHNP/PA-C, licensed to practice in North Carolina.
Bachelor's degree from an accredited college or university.

Strong interpersonal and communication skills
Self-motivated and able to work independently or as part of a team

Compensation: This is a Fee for Service (W2) position, the range for this position is $130,000-$187,000 per year based on 25-35 clinical hours per week.

Current approval by the Board of Nursing to perform medical acts.

Master's degree of Nursing specializing in NursePractitioner programs.
Physical Requirements

Pushing/Pulling 70+ pounds when administering patient care

Depth perception when performing medical procedures
Fine motor skills for grasping and controlling medical equipment, and to perform precise procedures such as sterile dressing changes
Travel Requirements:
This position requires regional travel a minimum of 4 days a week.
Travel maybe scheduled last minute at times if coverage is needed.
May require overnight stays
Work Environment:
May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.

We are looking for individuals who:
Reside in one of the following states: AK, AZ, CO, FL, ID, KS, NC, MD, NM, NV, NY, OH, OR, WA, WY
Have an active NP license + PMHNP certification in your state of residency
Have at least 1 year of experience regularly prescribing buprenorphine-based medications in an independent, outpatient setting as a core part of your daily practice (not something done occasionally or only during training)

About the Role
Clinicians work directly with patients to provide medication-assisted treatment for Substance Use Disorders (SUD) and empower patients in their individual recovery journeys as a member of Boulder's multidisciplinary care team.
This is a high-impact, patient-facing role, which involves delivering compassionate and evidence-based care through telehealth. Care is provided largely through standard audio-visual connection but also includes occasional audio/telephone visits and a small volume of text-based communications. Clinical work is assisted by Boulder clinical decision support tools within our custom EHR, staff education, and real-time support from colleagues and clinical leadership.
Success in This Role Patient Facing (90%)
Evaluate, diagnose, treat, and manage ongoing care for individuals with complex substance use disorders (SUDs) and related comorbidities
Understand, embrace, and incorporate harm reduction practices into all patient care
Help elicit patients'
own
goals for their recovery and help them in achieving these goals over time through therapeutic alliance and shared decision-making
Educate patients on Boulder's model of care, SUDs, related comorbidities, recovery strategies, and safer use where appropriate
Coordinate with teammates and Clinical Leadership to develop and oversee treatment plans, adjusting them as needed
Provide patient care during scheduled part-time shifts, which may include some nonstandard hours depending on coverage needs and candidate availability
Use technology effectively to facilitate a safe & rewarding patient experience
Interpret and share clinical results with patients without judgment in order to help them meet their goals and stay safe
Enable care coordination, connecting patients to clinical resources within and outside of Boulder through referrals where necessary, working with Peer Recovery Specialists, Case Managers, Care Advocates, and Care Navigators to help patients access social resources and navigate the healthcare system
Administrative (10%)
Deliver proactive, clear, and constructive feedback and collaborate with Boulder's Technology and Operations teams to troubleshoot as we continuously build out our digital treatment platforms
By the end of shift, all visit documentation must be completed, including all relevant information to enable consistent, high-quality team-based care
Review relevant Slack channels for updates and team communication
Understand and follow Boulder's policies and procedures
Requirements for the Role
Minimum 1 year of addiction treatment experience as an NP including adult populations
3 or more years of clinical experience as an NP or RN preferred
Degree from an accredited school of nursing and a graduate-level Clinician program
Ability to gain approval for multiple state licenses and insurance company contracts (no significant legal or credentialing barriers/sanctions)
Demonstrated expertise managing SUDs within a chronic care model, utilizing medication assisted recovery paired with social needs management in a Clinician capacity
Proficiency in electronic medical records and video conferencing; experience with Google Suite or similar platform highly preferred
Demonstrated ability to work effectively both independently and as part of a team; exceptional communication, interpersonal skills, and collaborative problem solving.
Ability to quickly learn and adapt to new technologies, workflows and evolving clinical practices within our growing organization
Ability to efficiently manage time and caseload within a part-time schedule
Comfortable and effective working in a fast-paced, team-based care model, maintaining focus and quality even with limited weekly hours
Dedicated, private workspace with a lockable door and high-speed internet to ensure HIPAA compliance
Cannot have previously been a patient at Boulder Care
Schedule Expectations Part-time:
Part-time clinicians typically work between 4 and 25 hours per week
Each clinician has one recurring weekly anchor shift, usually a weekday evening from 5pm to 9pm Eastern (2pm to 6pm Pacific)
All shifts are a minimum of 4 hours
Clinicians can pick up voluntary shifts as they become available, depending on business need and availability, including additional weekday evenings or morning and midday hours
Part-timers have the option to transfer to full-time roles when positions open and if it aligns with their goals
Compensation Part-time:
$68 - $80 per hour, depending on experience and qualifications
