Essential Functions & Responsibilities
Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF.
The provider is responsible for rounding daily and prescribing medications and treatments for residents residing at SNFs.
Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
Provider is responsible for timely documentation of all patient care/encounters
The provider will be available for phone/secure messaging consultations with facility nursing staff.
Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy?.
PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems.
Provider is expected to be on-site on and on time for their in-person scheduled shifts.
Timely attendance is imperative.
Reliable transportation required.
Shift Structure
Rounding time is 7AM-1PM local time, during which the provider must be available to nursing staff and address acute/emergent issues that arise.
Shifts begin around 7AM local time with flexibility within reason.
Shifts are daily Monday-Friday
Charting/documentation can be completed off-site.
This is an in-person position.

Your Qualifications:
Board Certified by the ANCC as a PMHNP

Under the direction of physicians, the Physician Assistant (PA) Surgical provides comprehensive care to patients, such as assessments, specimen collection, and consultation services including education on preoperative and postoperative care. * Provides comprehensive patient assessments including ordering, conducting and interpreting diagnostic tests; diagnosis; prescription of pharmacologic agents and non-pharmacologic therapies to both inpatients and outpatients. Interpreting highly complex surgical pathology, and relaying results to patients.
* Assist physicians in ER consults and floor consults, while in clinic and in between surgical cases. Determines if surgery is necessary, arranges all perioperative orders and scheduling cases. Provides preoperative and postoperative care including patient education, by maintaining communication with patients and their families.
* Assists the surgeon during operations by opening locations for surgery, operating laparoscopic cameras, suturing wounds, using surgical staplers, providing suction or anything else that the surgeon may need.
* Assists surgeon by maximizing patient volume, monitoring, and contacting high risk patients, returning patient calls in a timely manner, and providing follow up. Coordinates with other departments for continuity of patient care. Interprets radiology results to determine surgical plan.
* Develops with the patient and/or caregivers, a mutually acceptable cost-aware plan of care that maximizes health potential. Formulates a treatment plan which includes ordering diagnostic tests, selecting appropriate pharmacologic and non-pharmacologic interventions, develops a patient education plan, and obtains appropriate consultation or referral.
* Consults with physician appropriately regarding patient care issues and concerns. Communicates clinical findings to, and works collaboratively with, physicians to develop patient treatment and education plans.
* Performs procedures independently in the office. Conducts wound care for ongoing surgical wounds, requiring debridement in the office setting.
* Reviews all referrals for soft tissue tumors, skin lesions and properly delegates to self or surgeon for excision. Completes all discharge summaries for physicians.
* Demonstrates productivity and initiative by organizing patient care, delegating appropriate staff, prioritizing work load and seeking opportunities to improve clinical processes and staff/patient education.
* Hosts floor specific educational seminars for nursing education in the hospital setting. Helps implement new protocols and delegates tasks to floor nursing managers in relation to surgical care.
Minimum Qualifications:
* Masters Degree in Physician Assistant Studies and a maintained board/association certification

Your Role & Responsibilities
Provide comprehensive care to seniors in accordance with clinical standards.
Collaborate daily with your care team to ensure coordinated, high-quality care.
Manage referrals, hospital/SNF coordination, DME, and home health services.
Participate in daily huddles. CAs tend to lead the huddles
Document care accurately and efficiently with support from quality-based coders.
Participate in on-call rotation and support growth initiatives at your center.
Ensure compliance with all licensing and accreditation requirements.

What We're Looking For
Required:
Master's Degree in Nursing or completion of a PA program with board certification.
Active, unrestricted NP or PA license in the state of practice.
Strong communication and interpersonal skills.
Passion for value-based care and integrated team-based models.
Basic computer proficiency (EMR, email, etc.).
Preferred:
Experience in value-based care, primary care or Medicare Advantage settings.
Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
Familiarity with HEDIS quality indicators and Medicare guidelines.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$115,200 - $158,400 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.

Job Description
The PsychiatricNursePractitioner (NP) is a licensed registered professional nurse with graduate preparation (Master's or Doctorate) from an accredited NP program. This role is responsible for conducting psychiatric patient appointments and performing routine psychiatric examinations to ensure patient mental health, including speaking with patients about healthy lifestyle choices, completing diagnostic tests to help diagnose and treat psychiatric disorders, coordinating with other healthcare professionals to maintain updated medical records, and treat the whole person. This role will provide general and preventative psychiatric care, conduct mental health check-ups, treat mental health illnesses, order lab tests, prescribe medication for children and adults, and remain available in-between patients and during admin time for occasional Integrated Behavioral Health questions and consults.
ESSENTIAL JOB RESULTS
Providing health promotion and maintenance, disease prevention, independent psychiatricnursing diagnosis, and treatment of acute and chronic psychiatric disorders.
Developing and managing the psychiatric plan of care for patients or clients, based on the authorization for collaborative practice.
Ordering and interpreting diagnostic and therapeutic tests relative to patients' age-specific psychiatric needs.
Prescribing appropriate pharmacologic and non-pharmacologic psychiatric treatment modalities.
Providing mental health care for individuals by managing psychiatric problems encountered by patients and clients.
Providing innovation in evidence-based psychiatricnursing practice based upon advanced clinical expertise, decision making, and leadership skills.
Serving as a consultant, researcher, and patient advocate for individuals, families, groups, and communities to achieve quality, cost-effective psychiatric patient outcomes and solutions.
Collaborating with multidisciplinary team members by making appropriate referrals.
Facilitating staff, patient, and family decision making by providing educational tools.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Master's Degree in Nursing or Doctorate in Nursing Practice (DNP).
APRN license required for Kansas and/or Missouri depending on place of practice.
Registered Nurse in State of Kansas.
Certification as appropriate to psychiatric advanced or specialized practice by the American Nurses Credentialing Center or an equivalent body, or must be actively seeking certification. Certification must be successfully completed within one year of hire.
Current BLS certification.
Minimum of 1-2 years of clinical psychiatric experience.
