The part-time faculty will report directly to the CCPA Program Chair and indirectly to the Director of Didactic Education and will support student learning on campus within the innovative case-based small-group learning model.

The responsibilities of the part-time faculty will include, but will not necessarily be limited to, the following.
Support the delivery of in-person didactic courses 1-2 days per week, depending upon the contract, through the facilitation of student learning in small groups within our learning theater on the Santa Maria campus.
Collaborate as a member of the didactic faculty to achieve a shared vision for becoming recognized as the principal source of PAs from historically underrepresented groups providing culturally humble care to medically underserved communities.
Support the implementation of an innovative curricular model emphasizing blended learning.
Serve as a member of the dynamic team of didactic faculty to educate a highly diverse student body selected for their commitment to the CCPA mission using a holistic admissions process.
Contribute to the implementation of a comprehensive assessment strategy for the pre-clinical phase of the program.
Help to ensure students are adequately prepared to achieve programmatic assessment benchmarks (such as PACKRAT I, PACKRAT II, and PANCE).
Promote ongoing compliance with ARC-PA accreditation standards and best practices in contemporary physician assistant didactic education.
Requirements
The successful candidate will have the following qualifications, skills, and accomplishments:
Prior academic experience in medical education is preferred.
Substantial previous clinical experience is preferred.
Experience and/or concurrent employment within Community Health Centers and/or one of the CCPA local partner clinical sites, where the clinical education of our students is occurring, is preferred.
Applicants who can articulate how they can collaboratively contribute to the program's mission and vision will be given preference.
Applicants who can articulate their commitment to diversity, equity, and inclusion will be given preference.
Effective communication and collaboration skills are required.
Skilled in working with a variety of different student learning styles and collaborating with faculty colleagues.

JOB SUMMARY
The Hospice/PC NursePractitioner (NP) is a master's prepared advance practice registered nurse licensed in the state of California. The NP utilizes assessment and health care management skills in conjunction with the AHH Standardized Procedures and the collegial physician-nursepractitioner relationship to meet the health care needs of seriously or terminally ill patients. The NP provides expertise in hospice and palliative care as a nursing team leader and member of the interdisciplinary team. The NP is responsible for providing guidance and leadership to the team providing palliative care.
DUTIES AND RESPONSIBILITIES:
Under AHH Standardized procedures for NursePractitioners, provides medical care to seriously or terminally ill patients in conformance with the patient's IDG plan of care and collaboration with the attending MD and Medical Director:
Performs history and physical exams
Diagnoses and treats medical problems
Orders and interprets diagnostic and laboratory tests, as appropriate
Educates and counsels individuals, families and groups
Facilitates appropriate health maintenance activities (limited immunizations)
Provides the skilled nursing services necessary to adhere to the patient's plan of care developed by the IDG including the patient's physician and family
Ensures adherence to the plan of care for every patient/family assigned to the team
Performs assessment and identifies the patient's and family's physical, psychosocial, environmental, safety and developmental needs and individualizes the patient's plan of care based on the needs identified
Assesses all aspects of the patient's pain and develops an individualized pain management plan
Anticipates prevents and treats undesirable symptoms, secondary symptoms and other comfort needs
Supports, instructs, and educates the patient, family, caregivers and facility staff as necessary to meet the patients needs
Documents problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes achieved from intervention and care provided
Maintains the dignity and supports the unique beliefs of the dying patient and their family
Provides holistic, family-centered care across treatment settings to improve quality of life

ComplexCare Solutions (CCS) specializes in helping those in need by delivering high value in-home and virtual assessments for health plans nationwide. We pride ourselves on our member engagement aimed towards evaluating current health status, gaps in care, potential health risks and care management opportunities with a focus on driving better clinical outcomes. Our visits give Providers the opportunity to spend a full hour with these members which allows them to capture a comprehensive picture of that individual's personal needs and what is required to keep them in the home and out of the hospital.

Pay Range:
In-Home Assessment: $140
Telehealth Assessment: $85
This position is eligible to participate in a company 401K plan providing the opportunity to save for retirement through employee contributions. New hires will be automatically enrolled in the company 401(k) plan at a 3 % contribution rate with the option to opt out.
Responsibilities
Knowledge of CMS Regulations and NCQA HEDIS Guidelines;
Knowledge of Evidence-based Clinical Practice Standards: American Diabetes Association (ADA), American College of Cardiology (ACC), American Heart Association (AHA) Familiarity with ICD-10 and CPT-4 coding practices;
Complete comprehensive, accurate and thorough review of the assigned member population, including timely completion and submission of all required encounter documentation (paper or electronic);
Ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS/DHHS, Company policy, and client requirements;
Support the physician/patient relationship and ensure timely and adequate communication, documentation of assessment findings, recommendations, need for additional services, emergency services required if necessary and need for follow up and timelines for follow up to primary care provider (PCP) and health plan as required;
Make general recommendations to members intended to improve members' knowledge of their chronic condition(s), such as information concerning recommended testing;
Address and close identified gaps in care (disease-specific or preventive);
Recognize emergent or urgent situations requiring escalation and take appropriate action as specified in Company policies, and as determined by reasonable professional judgment and ethical professional practice standards; and
Perform, document and communicate results of Point of Care (POC) Testing.
Maintain compliance with Company policies, procedures and mission statement;
Adhere to all confidentiality and HIPAA requirements as outlined within CCS's Operating Policies and Procedures in all ways and at all times with respect to any aspect of the data handled or services rendered in the undertaking of the position;
Fulfill those responsibilities and/or duties that may be reasonably provided by company for the purpose of achieving operational and financial success of the Company;
Uphold responsibilities relative to the separation of duties for applicable processes and procedures within your job function;

Qualifications
Active un-encumbered license to practice nursing;
NursePractitioner Certification from either the ANCC or AANP; in Family, Adult, Gerontology or Emergency Medicine.
Board Certified through the AANP or ANCC
Maintains current CPR certification
Compliance, prior to hire, with recommended Healthcare Personnel Requirements for vaccinations and preventive testing:
Hepatitis B
Influenza
MMR: Measles, Mumps and Rubella
Pertussis, Tetanus and diphtheria and acellular pertussis (Tdap)
Varicella
Tuberculosis
Home Health exp a plus
Must be able to effectively communicate with elderly and chronically ill patients and families;
Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial;
Ability to multitask;
Excellent customer service skills;
Bi-lingual or multi-lingual a plus.
