Job Duties:
Examine, diagnose, prescribe, and carry out services and treatment plans.
Collaborate with general dentists on referral cases, as well as all other clinical and non-clinical personnel as necessary.
Uphold the policy, protocol, and procedures which follow the most current accepted professional standards.
Improving clinical skills and acumen through participation in continuing education and training opportunities (continuing education support is available).
Participate in various community outreach initiatives as necessary.
Required Experience:
DDS/DMD from a dental education program accredited by the Commission on Dental Accreditation.
Complete a Pediatric Residency accredited by the Commission on Dental Accreditation or have at least 5+ years of experience focused primarily on pediatrics.
Current, valid license to practice dentistry in state where providing care (License must be in good standing) or eligible for licensure.
Other certifications as required by state to include- CPR, DEA, etc.

Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
Notifies the supervising RN with requests for re-authorization of “more visits needed” before third-party insurers' authorized number of visits expires.
Suggests Community Services that may be available to the patient to assist in safe home care needs.
Participates in the Performance Improvement Committee process as requested.
Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function.
Participates in the growth of the office by being a willing preceptor for newly hired same-discipline employees.
Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the patients serviced.
Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event's occurrence.
Submits written time requests 2 weeks or more in advance of planned time off.

Nature and Scope

Nursepractitioners are primary health care providers who practice in ambulatory, acute, emergency room, critical care and long-term care settings. According to their practice specialty, these primary care providers provide nursing and medical services to individuals, families, and groups. In addition to diagnosing and managing acute episodic and chronic illnesses, nursepractitioners emphasize health promotion and disease prevention. Teaching and counseling individuals, families, and groups are a major part of nursepractitioners role.
The incumbent must provide optimal patient care through assessment, planning, implementation, and evaluation of neonatal, adolescent, adult, and geriatric patients and families, as applicable. Demonstrates the knowledge and skills necessary to provide care, based upon physical, motor/sensory, psychosocial, and safety appropriate to the age of the patient served. A Nursepractitioner performs the following acts in addition to the ordinary functions of a registered nurse if he/she is properly prepared and the acts are currently within the standard of medical practice for his specialty and appear in their protocols:
1. Systematically assess and triage the health status of persons and families by:
a. Obtaining, recording, and interpreting medical histories
b. Performing physical examinations as deemed necessary based upon the clinical scenario
c. Reviews/orders/interprets diagnostic tests and radiographic results, and appropriately acts upon the results of those tests
d. Performing or initiating selected diagnostic procedures.
2. Based on the information obtained in the assessment of a person's health, the nursepractitioner will manage the care of common, acute, recurrent, or long-term health problems. Management may include:
a. Initiating a program of treatment for the patient, in conjunction with the supervising physician or independent of physician supervision in select situations.
b. Prescribing medications consistent with Nevada prescribing laws and DEA privileges.
c. Evaluating responses to health problems and programs of treatment.

f. Modifying programs of treatment based on the response of the person or family to treatment.

Job Summary:
A Licensed Practical Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors and teaches and educates patients and their families according to the established professional plan of care. Responsibilities:
Maintains the nursing frequency established by the supervising RN.
Follows the plan of care established by the supervising RN.

Prepares equipment & materials for treatments observing aseptic techniques as required.
Performs home safety check and environmental check of the patients home environment on all visits. Reports any unsafe or potentially unsafe condition to the supervising RN.
Performs nursing visits and documents accordingly.
Reviews and follows the supervising RNs plan of care, communicating frequently with the supervising RN on any suggested changes necessary.
Collaborates with the supervising RN on a weekly (or more frequent basis) to benchmark toward the established outcome goals.
Review the patients ADL and ADL abilities.
Teaching patients & family self-care techniques.
Communicate any suggested changes to the supervising RN.
Reinforces the HHA plan of care when HHA services are ordered.
Effectively communicates with patient/family the POC and progression of such. Keeps the patient informed ongoing.
Effectively communicates with other disciplines in the case (actively participates in case conferencing) to effectively and appropriately problem-solve as situations arise.
Communicates effectively with the supervising RN, scheduled visits, and changes to the schedule every week. The caseload is self-scheduled but communication of the clinicians schedule to the nursing supervisor is essential.
Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual patients needs.
Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
Notifies the supervising RN with requests for re-authorization of more visits needed before third-party insurers' authorized number of visits expires.
Suggests Community Services that may be available to the patient to assist in safe home care needs.
Participates in the Performance Improvement Committee process as requested.
Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function.
Participates in the growth of the office by being a willing preceptor for newly hired same-discipline employees.
Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the patients serviced.
Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the events occurrence.
Submits written time requests 2 weeks or more in advance of planned time off.
