The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
Performs other duties as assigned and modified at manager's discretion.

Position Summary:
The psychiatricnursepractitioner will provide care in-person or via videoconferencing to child and adolescent patient populations in outpatient care, including partial hospital and/or intensive outpatient programs.
Qualifications include:
Minimum of two years of experience as a psychiatric provider is preferred
Graduate of an accredited Master of Science in Nursing (MSN) program with certification as a PMHNP
Current and unrestricted Texas APRN license
Ability to obtain and maintain licensure in multiple states as needed
Valid DEA license or ability to obtain one in multiple states
Active CPR certification
Experience delivering high-quality, patient-centered care
Strong clinical judgment and ability to make decisions based on patient presentation
Experience in telemedicine is preferred
Proficient in navigating and documenting independently in an EMR
Comfortable using Microsoft 365 suite, Zoom, and other computer tools
Excellent verbal communication and customer service skills
Able to sit for up to 8 hours during scheduled shifts
Flexible within defined work hours and adaptable to changing environments
Demonstrates a desire for continuous learning and professional growth
Reliable, professional, and able to work independently with minimal supervision
Job Duties Overview:
Provide psychiatric evaluations, diagnoses, and treatment plans for patients in an outpatient setting.
Interpret lab results and incorporate findings into individualized care plans when appropriate.
Make independent clinical decisions in alignment with established protocols and national guidelines for treating mental health conditions.
Maintain strong, professional relationships with patients and referral sources to ensure continuity of care.
Ensure a positive patient experience during in-person and virtual appointments, addressing concerns and promoting satisfaction.
Log into the videoconferencing platform and electronic medical record (EMR) at least 15 minutes prior to each scheduled virtual shift to confirm system readiness.
Promptly report any connectivity or technical issues to the designated care coordinator and IT Help Desk to minimize disruptions to patient care.
