The general duty of the Psychiatrist is to provide covered professional psychiatric services to enrolled patients of the Veterans Affairs Medical Center (VAMC) in accordance with the terms and conditions of the Veteran Affairs Mental Health Program, and to act as a collaborating physician for the onsite Psychiatric Mental Health Advanced Practitioner(s). ESSENTIAL FUNCTIONS: Provide treatment for patients with general psychiatric disorders in a variety of treatment settings, including tele-mental health. Serve as subject matter expert in support of the behavioral health interdisciplinary team with responsibilities of providing clinical education, coordinating consults and triaging specialty clinic referrals. Collaborate with approved fee-based community mental health inpatient programs to assist/coordinate discharge planning, medication, and follow-up appointments, as well as, coordinating with ambulatory care services, as appropriate. Develop and implement treatment plans for a range of mental illnesses such as anxiety, depression, personality, and behavioral disorders, phobias, post-traumatic stress, and more. The position is directly linked to customer service and the Service Excellence program initiatives. The general duty of the Registered Nurse Care Manager PCMHI is to provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) and to manage patient care. ESSENTIAL FUNCTIONS: Provide nursing care based on physician, mid-level provider and CBOC Clinic Manager's orders, to meet the needs (physical, mental, and emotional) of patients. Conduct patient work-ups for medication management by the Mental Health provider. Manage mental health care for Veterans in coordination with the Mental Health team. Complete mental health reminders and documentation during patient visits. Perform EKGs for the medication management Mental Health provider, as needed. Triage and screen Veterans contacting the clinic via phone, walk-in, or secure messages for mental health concerns. Accept warm hand-offs from Primary Care for Veterans needing additional mental health screening and evaluation. Supervise support staff, if applicable, in delivery of care to patients. Lead and participate in PACT team huddles. Work with the Primary Care Physician (PCP) to ensure the clinic is adequately staffed for quality care delivery and the promotion of continuity of care between the VAMC and the clinic. Maintain universal precautions and infection control practices. Supervise medical assistants and/or licensed practical nurses who obtain initial information from patients (i.e.: vital signs, symptoms, and complaints) through interviewing patients and reviewing record. Demonstrate, understand and perform triage activities for all walk-in patients and medical phone inquiries.
