The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF) and home settings depending on the nature of the assignment or providing assessments to members in SNF and home settings. Serves as patient advocate. Patient management includes the following: Writes admission, transfers and discharges orders. Orders and interprets appropriate laboratory and diagnostic studies. Orders of appropriate medication and treatments. Refers patients for consultation when indicated i.e. dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc. Documentation through in-depth progress notes and summaries. May perform invasive procedures independently upon the completion of documented competency. Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team. Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs. Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary. Uses advanced communication skills to problem solve complex situations and to improve processes and service to patients. Other Responsibilities may include: Collaborates with other multidisciplinary team members to analyze and evaluate current systems of health care delivery to identify and implement new practice patterns as appropriate. Participates in outside activities that enhance personal and professional growth and development. Initiates arrangements and writes orders for SNF discharges and completes appropriate paperwork. Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as soon as medically appropriate. Introduces self to patient/family and explain nurse practitioner role. Facilitates patient/family conferences to review treatment goals, optimize resource utilization, provide family education and identify post-SNF needs. Enhances a collaborative relationship to maximize the patient's/family's ability to make informed decisions re: goals of care, palliative care and hospice. Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required. Prescribes medication to patients based on State of practice. Other duties as assigned and modified at manager's discretion. Provides medical, diagnostic and treatment services to individuals with substance use and mental health disorders in collaboration with agency psychiatrists. Work is performed under general supervision and is guided by policies and procedures, quality assurance, public and private payor guidelines, diagnostic guidelines and professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines. Primary role will be serving those with a primary opioid use disorder in a mobile OBOT/OBAT setting. DUTIES AND RESPONSIBILITIES Performs mental status and/or other medical examinations in collaboration with the psychiatrist. Medical examinations include those for persons admitted to the agency's crisis stabilization program within one business day of their admission. Diagnose illnesses and carries out the necessary medical or psychiatric treatment program for adolescents and adults served by the agency, including assessment and induction of medication for opioid use disorders-excluding methadone. Provides technical supervision of direct treatment services performed by other service providers. Provides consultation, education and training services to individuals and agencies. Provides crisis intervention/emergency services, sharing on-call responsibilities with other prescribers in order to provide necessary coverage during nights, weekends and holidays. Conducts psychiatric evaluations as requested by appropriate individuals/agencies. Performs timely evaluations and provides treatment and referral to persons discharged from state and private psychiatric facilities in accordance with continuity of care agreements. Provides required medical and administrative documentation. Performs other related duties as assigned. KNOWLEDGE, SKILLS, AND ABILITIES Thorough knowledge of the principles, standards, practices and techniques related to Psychiatry and nursing; thorough knowledge of the characteristics and precautions to be taken in the administration of psychotropic medication; thorough knowledge of the principles, practices, and techniques related to individual therapy, counseling practices and risk assessment for the mentally ill; thorough knowledge of individual manage ment guidelines for Virginia; thorough knowledge of the laws and legal procedures relating to hospital admission; thorough knowledge of community resources hospitalization options, treatment providers and other agencies in service to the mentally ill population; ability to establish and maintain rapport with the individual population; ability to establish and maintain effective working relationships with other staff and with private and public community partners; physicians, subor dinates and others contacted in the course of work; ability to function calmly in stressful, if not crisis situa tions; ability to risk assess and prioritize service demands; ability to prepare and maintain patient medical records and prepare reports in compli ance with licensure regulations; ability to conduct effective interviews, observe, assess and record individual behaviors and functioning and develop treatment and service plans; ability to communicate effectively, orally and in writ ing; ability to confidently provide expert testimony in both oral and written formats; ability to operate a vehicle. TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE Master's degree in nursing and at least three years professional clinical experience in community mental health, with at least one year treating those with opioid use disorders. LICENSURE AND CERTIFICATIONS Certification as a psychiatric nurse practitioner through the Virginia Board of Nursing. Current DEA registration. Valid VA driver's license. OTHER Must possess all qualifications and skills required by federal and state regulations to bill Medicaid for the services provided. ENVIRONMENTAL AND PHYSICAL EXPOSURE Work involves some exposure to disagreeable environmental conditions such including the requirement to intensely interacting with challenging and potentially violent individuals having unsanitary behaviors, poor hygiene and/or contagious illness. This position will primarily be performed in a mobile clinic environment. RELATIONSHIPS Relationships require advanced communication, social and interpersonal skills, tact and the use of specialized techniques to elicit information from and convey information to clients including hostile and abusive individuals and client families; represent the program and obtain cooperation with clinical professionals within and external to the agency.
