The responsibilities of the optometrist include:
5 full days weekly (Monday through Friday):
Perform detailed medical histories and comprehensive, dilated, undilated and postoperative eye exams. for all patients
Request and review records from previous evaluations to include OCT images and reports, serologic testing, and previous eye exams
Perform refractions and prescribe corrective lenses and medications as necessary
Assist with intravenous access for injections of fluorescein for angiography purposes under the supervision of an ophthalmologist.
Facilitate completion of documentation including results of sensorimotor exam, external and fundus photography, Humphrey Visual Field, and Ocular Coherence Tomography
Analyze test results and develop a treatment plan
Consult with and refer patients to other health care practitioners as necessary
Participate in department research initiatives and/or conduct provider initiated research projects
Assist in teaching and lecturing responsibilities for our ophthalmology residency program and future optometric residency program
The ideal candidate will have residency training in ocular disease or primary care or at least two years' experience in treating related disorders. At the time of application candidates must have an OD degree from an ACOE accredited institution and be licensed in the State of California with therapeutic drug prescribing certification (TPA) glaucoma certification (TLG). Residency training and Clinical teaching experience are highly preferred but not required. Low vision training and experience also highly preferred. The level of appointment will be determined by the experience of the applicant. Salary is commensurate with experience.

Compensation & Benefits
Expected Total Annual Compensation: $130,000 - $175,000
The compensation range listed above includes variable compensation and is based on a 1.0 FTE work schedule/effort, as outlined in the individual department compensation plan. The compensation range for starting base compensation only, which is adjusted in accordance with years of experience and/or faculty rank, is expected to be $100,000 - $140,000. More information on compensation is discussed with the department during the recruitment process.

Risk assessment for Members with Diabetes, as per the American Diabetes Association (ADA) guidelines.
Patient education and management.
Education of the Primary Care Physician (PCP), Case Manager (CM), or referring Physician on current wound management practices.
Provide patient updates to PCP, CM, and Physicians.
Communicate in a clear and understandable manner using appropriate verbal and non-verbal styles.
Actively participate in departmental activities to improve training methods and achieve company goals.
Assist with one-on-one training and coaching sessions as assigned.
Attend required meetings and participate as a committee member as requested.
Assist with special projects as needed.
Maintain strict confidentiality.

Qualifications
Candidates must hold a current state license as a Nurse Practitioner or Physician Assistant. Experience in direct patient care, particularly in the homecare setting, is highly valued. Strong clinical acumen, excellent communication skills, and a passion for innovative wound treatment are essential.

Job Summary

Essential Duties/Responsibilities
Possess comprehensive behavioral health skills to include, but not limited to mental health assessments; screenings and triage; pharmacological management; therapy/crisis intervention; organization and facilitation of psychoeducational and support groups; short-term case management; documentation; and referrals
Perform appropriate diagnostic and clinical assessments in evaluating patient behavioral health and medical care concerns
Identify, prioritize, and manage complex behavioral health and co-occurring problems
Develop treatment plan in collaboration with the patient while monitoring their progress
Work as part of a multidisciplinary team in collaboration with doctors, clinicians, nurses, medical assistants, case workers, education professionals, etc.
Communicate and collaborate professionally and effectively with patients, healthcare professionals, service agencies, etc.

Compensation & Benefits
The compensation range listed is for starting base compensation only and is adjusted based upon years of experience and/or faculty rank: $250,000- $321,000. This amount does not include variable compensation or extra productivity and is subject to the individual department compensation plans. More information on compensation is discussed with the departments during the recruitment process.
