Cleveland Clinic's Digestive Disease and Surgery Institute is currently seeking a Physician Assistant or Nurse Practitioner (Acute Care Certified) for a full time position with Liver Transplant. This is not a surgical position. This position would cover a medically complex patient population which would include: adults post-liver transplant in the ICU, inpatient regular nursing floor, as well as cross-coverage of the inpatient pediatric liver transplant patients. This provider will be responsible for the overall medical management of the patient post liver transplant as well as inpatient liver transplant surgical consults. There will also be coverage of the outpatient liver transplant clinics as needed. Daily virtual or onsite visits to local LTACs and subacute facility will also be required on a rotational basis. The hours for this position will be four 10 hour shifts per week to include occasional weekends. Coverage and hours may change depending on practice needs. Administer somatic therapies, including prescriptions and periodic review of medications, including drug interactions and medication side effects
Monitor the physical health of agency clients using appropriate labs and evaluation of movement disorders.
Perform assessments of mentally ill adults through reviewing clinical records and attaining collateral information.
Lead interdisciplinary treatment team(s).

When you begin seeing patients on your own, you will initially have an hour scheduled for each visit, allowing you extra time to get to know your patients, review charts, and complete your notes.
After your initial meeting, existing patient follow-up visits are normally scheduled at 30-minute intervals, and psychiatric evaluations are scheduled for one hour.

Possession of an M.D. or D.O. degree and successful completion of an approved Board of Psychiatry Residency Program.

Reporting to the Associate Medical Director at their location and under the oversight of the Medical Director, Psychiatry and the Chief Medical Officer, a Child Psychiatrist at Signature Health, Inc. provides compassionate, supportive, patient-centered psychiatric medical care to our patients. Our Child Psychiatrists work diligently to help improve the daily lives of our patients by providing evaluations/consultations, diagnosis, monitoring of progress, and pharmacologic and other appropriate psychological health treatment services. Our Child Psychiatrists work collaboratively with Signature Health staff, including our primary care and other non-psychiatric medical care teams members, counselors, case managers, and other outside providers to provide quality holistic care for our patients.
HOW YOU'LL SUCCEED
Provide ongoing consultation and management for assigned patients as requested or as necessary in accordance with accepted standards of psychiatric practice.
Provide psychiatric services on the days and at the times established and scheduled by Signature Health. Our expectation is that the Child Psychiatrist's schedule will be maximized during his/her clinical schedule in order to best serve all our clients in need.
Utilize the electronic medical records system to timely and accurately fulfill all administrative responsibilities attendant to his/her performance of direct patient care, including but not limited to, entry and completion of progress notes detailing all patient encounters, services provided and treatment plans recommended; electronic order entry of medications and diagnostic tests; and other required documentation in accordance with the policies and procedures of Signature Health and/or other relevant accrediting or regulatory bodies. Psychiatrist must ensure compliance with Signature Health's note completion policy.
Prepare and attend to all reports, claims, and correspondence (including email) necessary or appropriate to the performance of the professional services. Email is a regular form of communication within Signature Health and therefore prompt response, generally a response within 24 hours (or sooner as medically/business appropriate) is required.
Refer patients to other services within the agency, other providers, including specialists and other health care providers, when and if needed.
Maintain competence in his/her area of specialty by attending professional conventions or seminars as well as participating in educational activities offered by professional societies on a reasonable basis.

Cleveland Clinic?s Digestive Disease Institute is currently seeking a qualified Physician Assistant or Nurse Practitioner (Adult or Family certified) for a full-time position in Outpatient Gastroenterology at our South Region locations (Medina Hospital and Summit Gastroenterology). This provider will treat General GI patients. Some common diagnosis would include Abdominal Pain, GERD, IBS, Colorectal Cancer Screening, GI Bleeding, Biliary disorders, Diarrhea, Constipation, Nausea, Weight Loss, Bloating, Inflammatory Bowel Disease, Abnormal liver function tests, Fatty liver, and Hepatitis. Some typical duties will include Outpatient office visits, rectal exams, responding to patient calls/messages, interpreting diagnostic testing and discussing results with patients, collaborating with supervising physician(s) as needed. The hours for this position will include days. Currently no nights or weekends are required but this could change in the future depending on department needs. The ideal candidate will be comfortable with a high level of autonomy and working as a collegial member of the team. Experience is preferred, however new graduates with GI experience will be considered.
Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.
Responsibilities:
Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team.
Performs and interprets common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.
Performs routine and specialized procedures as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.
May prescribe and monitor medications as allowed by license through applicable state board and Cleveland Clinic policies.
Educates patients and answers questions regarding their disease, treatments, related drug and treatment side effects and hazards.
Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care.
Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment.
Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.
Participates in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care.
Facilitates appropriate length of stay, discharge planning and compliance with regulatory standards for inpatient management.
Supports professional development of the health care team.
Promotes translation of evidence-based practices, guidelines, and research.

Education:
Effective 05/01/2014: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
Graduate of an accredited APRN program.
If in neonatal care service area Master?s Degree is required upon hire.

Certifications:

Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA); or Neonatal Resuscitation Program (NRP) through the American Academy of Pediatrics (AAP) may be required based upon work setting.
Must have arrangements with at least one physician in accordance with requirements set forth by the State Board of Nursing in applicable states.
