We are growing in the form of teams composed of multiple therapists and a prescriber.
We emphasize regular staffing with your collaborating therapists, at least monthly, to discuss patient cases or clinical topics.
Clinical work is performed from home, and days and hours are per the clinician's choosing. Clinical team meetings are also virtual.
Compensation is per patient and per code billed, there is a medical, dental, vision, and short-term disability insurance benefit for full-time clinicians.

Essential Functions & Responsibilities
The physician serves as the attending physician for the residents residing at each facility;
The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes
Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasyâ„¢
PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems
Participating in Theoria's Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs
Physician must be able to supervise collaborating nurse practitioners and physicians assistants
Physician must be able to utilize electronic/online medical tools such as EHR systems and state/county medical systems utilized for Death Certificates and administrative functions.
Shift Structure
Shifts are flexible depending on physicians' schedule;
Typically, attending physicians work a minimum of 1-2 days/week at each facility they oversee;
Rounding times are flexible with no set start and end time. Preferably, physicians start in the morning between 7-10 a.m. and finish their day when their caseload has been seen;
No nights, or on-call
Call is covered by Theoria's virtual care team

Benefits & Compensation:

+ Responsible for all clinical aspects of perfusion services, including cardiopulmonary support, hepatic bypass, single-limb perfusion, rapid infusion systems, centrifugal pumps, LVADs, and RVADs. Also responsible for operating red cell salvage devices and providing perioperative blood product support.
+ Monitors and documents the anticoagulation status of the patient and medicates per established protocols. Administers and records blood products and pharmacological agents via the extra-corporeal circuit, following protocols outlined in the Physicians Preference Card.
+ Cleans and maintains perfusion equipment to ensure optimal performance and safety.
+ Documents all patient assessments, care delivery, and technical interventions in accordance with hospital policy and regulatory standards.
+ Maintains appropriate records in compliance with JCAHO, CLIA, AMSECT, and the American Board for Cardiopulmonary Perfusion recommendations.
+ Responsible for all clinical aspects of perfusion services for heart transplantation for locations that support heart transplant.
+ Performs heart and lung assist pump implantation, including short-term cardiopulmonary support (CPS) for emergent Cath Lab and bedside rescue; long-term CPS (ECMO) for post-cardiotomy heart failure and/or pulmonary distressed patients, including transferred patients; and RVAD/LVAD/VIVAD support for patients who fail to wean from CPB, are transferred post-surgery from other institutions, or require temporary support following VAD implantation.
+ Provides CPS/VAD training to other Intermountain Healthcare facilities, and trains and mentors other perfusionists.
+ Performs daily rounds and maintenance on CPS/VAD patients.
