What You'll Do:
Provide psychopharmacological consultation, assessment, evaluation and treatment for assigned caseload providing a range of psychosocial care.
Prescribe all necessary medications and treatments according to established practice guidelines and outcomes of care and makes necessary revisions in care plan.
Assess and monitor the person served for safety and efficacy with medication management.
Order, interpret and evaluate appropriate lab and diagnostic tests to assess their clinical health care needs. Based on the outcomes of diagnostic, lab and physical assessment findings, arranged for appropriate plan of care and follow up.
Communicate with treatment team and collateral contacts such as PCP, community agencies, and families as appropriate.
Maintain strong relationships with PCPs, families, and community partners; attend meetings as needed.
Collaborate with the medical director, if needed, in the coordination and provision of psychopharmacological treatment.
Provide direct care, counseling, and teaching to the person served and their families according to established collaboration protocols and standards of care.
Provide quality mental health care that is person-centered and goal-oriented with a focus on improving the person's functional level using shared decision making.
