In this role, you will represent ZEISS's interests in public and government affairs, focusing on trade policy, geopolitics, and key regulatory issues. This position will specifically represent high tech areas such as semiconductor equipment , ensuring alignment with the company's strategic priorities and advancing ZEISS's position in the U.S.

Serve as the primary point of contact for government-related inquiries and communications, both internally and externally.

Education: Master's degree (or higher) in public policy, law, economics, or a related field.
Experience: Minimum of 7 years of professional experience in public and government affairs, with a strong focus on trade policy, geopolitics, and related regulatory matters.

The annual pay range for this position is $135,000 - $165,000The pay offered for this role may be influenced by factors such as job location, scope of role, qualifications, education, experience, & complexity/specialization/scarcity of talent.This position is also eligible for a performance bonus or sales commissions. The Community Marketing Outreach Representative is responsible for generating local community brand awareness of the health plan with accountabilities in driving leads and sales (direct or indirect) through grass roots efforts. Zócalo Health is looking for a Community Health Worker to work directly with our patients to help them navigate their health and social needs. You will work with an assigned panel of members dealing with unmet social needs and numerous health conditions. You will educate patients on disease prevention and healthy behaviors, coordinate comprehensive care by scheduling appointments and facilitating follow-ups, and address social determinants of health by connecting patients with essential community resources. Play an active role in patient registration and enrollment, including organizing community engagement and outbound calls to patients.
Conduct outreach (virtually and in-person) to patients scheduled for appointments and complete initial intake.
Engage with a panel of assigned patients to provide care navigation, appointment logistics, prescription drug support, lab support, referral coordination, care plan adherence, and resource sharing.

What You'll Do

Medical Case Managers will utilize Motivational Interviewing (MI) techniques when engaging with clients. As appropriate, Supervisors will recommend and/or conduct MI trainings and Learning Groups. Supervisors and staff will review individual MI techniques and Supervisors will provide ongoing education and oversight with MI skills, as appropriate.
Participate in Equitas Health Committees and Performance Improvement Teams as appropriate and assigned by direct supervisor.
Prepare for and attend individual and group supervision per Supervisor's schedule. Non-Medical Case Managers are responsible for bringing client concerns, process questions, and other needs to scheduled supervisions. Non-Medical Case Managers are required to attend 5 hours of supervision per month.

Key Responsibilities (Bridgewell Program)
Conduct CHW-aligned assessments to identify social, medical, and resource needs for Medicare clients
Provide SDOH resource navigation (housing, food access, transportation, financial assistance, etc.)
Assist clients with obtaining medical, behavioral health, and community-based services
Coordinate care and follow-up support for clients with chronic illnesses or high-risk conditions
Encourage preventive care and adherence to care plans
Provide local education on healthy living, wellness, and navigating services
Document all client interactions, resource referrals, education, and outreach
Track client progress and assist with monthly monitoring requirements
Maintain accurate and timely documentation in the electronic health record
Communicate professionally with medical providers, clinics, and community partners
Support Medicare clients through wellness coaching, resource reminders, and navigation
Participate in periodic trainings and quality improvement initiatives (virtual)

Community Health Worker
The Community Health Worker focuses on direct support, outreach, and engagement to improve community health outcomes, focusing on improving health, well-being, and access to essential services. The Community Health Worker serves as a liaison between community members or patients and health and social service systems, providing culturally appropriate health education and addressing basic social drivers of health. Community Health Workers may work in a variety of settings, including serving in the community or assisting patients in physician practices based on specific market design.
Essential Job Functions
Builds trusting relationships with individuals and communities to mediate between individuals and health/social service systems. Identifies and engages individuals and families in need through outreach activities and community events.
Evaluates the specific needs and challenges faced by community members, including health, social, and educational needs. Identifies barriers such as housing, food insecurity, and transportation challenges, and connects individuals to appropriate resources.
May conduct outreach activities, including home visits and participation in community health fairs and local events, to build awareness and engage underserved populations.
Maintains accurate and up-to-date records to support advocacy efforts, monitor outcomes, and contribute to program evaluation.
May perform or assist with health education and/or basic health screenings, such as blood pressure and glucose checks, under appropriate supervision.
Provides support to ongoing community health programs.

Licensing/Certification
Community Health Worker (CHW) Certification in the state of practice (preferred; required within 12 months of hire)
