Job Description & Requirements Specialty: Hospice Discipline: RN 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title: Registered Nurse (RN) - Case Manager Work Location: Pueblo, Colorado Area Pay: $60/hr or $2,400 a week Schedule: Day shift, 8:00 AM - 5:00 PM No on-call, weekends, or overtime required Travel Requirements: Local travel within the Pueblo area Mileage reimbursement at $0.45/mile Reliable transportation and valid car insurance required Licensure & Experience: Active Colorado or Multi-State RN License (compact license accepted) Preferred: HomeCare HomeBase (HCHB) and Hospice experience Job Summary The Registered Nurse Case Manager is responsible for coordinating and delivering comprehensive nursing care to patients within an assigned territory. This role focuses on ensuring high-quality, cost-effective care while promoting patient independence and satisfaction. The RN collaborates with physicians, interdisciplinary team members, and community resources to support optimal health outcomes and smooth transitions of care. Key Responsibilities Conduct comprehensive assessments for assigned patients, identifying clinical and non-clinical needs. Develop and implement individualized care plans with measurable goals and interventions. Collaborate with primary care providers and other healthcare professionals to coordinate ongoing care. Monitor patient progress and modify care plans as needed to achieve desired outcomes. Facilitate care transitions and discharge planning in collaboration with physicians, social workers, and other stakeholders. Identify opportunities for health promotion and illness prevention. Maintain detailed documentation of patient assessments, interventions, and outcomes. Act as a patient advocate by ensuring privacy, dignity, and confidentiality in all interactions. Qualifications Associate's or Bachelor's Degree in Nursing (BSN preferred). Current, unrestricted RN license (Colorado or Compact State). Case Management certification preferred. Strong analytical, problem-solving, and critical thinking skills. Ability to manage multiple cases while maintaining attention to detail. Excellent communication and organizational abilities.
