What You'll Do:
Builds program specific caseload by seeking out referrals from providers and/or other identified partners to meet program objectives.
Conducts intake screenings to initially assess clients, identify their immediate needs, and determine appropriate YVFWC case management program services.
Conducts in-depth client interviews (in-person) utilizing a systematic social services assessment to fully determine current status, barriers and needs.
Refers clients to appropriate agencies/services based on the determined needs.
Assists clients with the self-referral process.

Creates, updates, and maintains each clients' service plan, and other documentation and files, in accordance with program guidelines and funding regulatory requirements.

Participates in meetings as needed, including interdisciplinary team case reviews.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA) and YVFWC requirements.
Qualifications:
Bachelor's Degree in a social service field such as Social Work, Behavioral Sciences, Psychology, or Child Development.
Associate's Degree with two years' experience working in community social services, public health services, crisis intervention, outreach or linkage programs.
High school or GED and four years' experience working in community social services, public health services, crisis intervention, outreach or linkage programs.

What You'll Do:

What You'll Do:
Executes incoming calls, assesses client/patient needs, and identifies appropriate services that are comprehensive and individualized to best serve each client/patient.
Collects appropriate documentation to support and determine program services eligibility.
Assists client/patient with scheduling dates for appointments or classes.
Processes patient referrals for a variety of services including Medical, Dental, and programs including Behavioral Health.
Monitors and documents active referrals and client participation.
Assists patients with completion of applications and forms required by the program(s) they will participate in. Ensures patient information is complete and entered in appropriate data system.
Responds to basic questions from patients.
Provides follow-up contact with patient regarding service and referrals recommended. Encourages client participation and retention in various programs and classes.
Performs work to align with productivity goals and standards including # patients, # encounters, # appointments scheduled and # classes facilitated.
Provides program administrative support such as data entry, client file management, report development and contract compliance.
Coordinates and promotes YVFWC programs. Recruits clients/patients for dental services, medical services, social services or education classes. Reserves conference rooms, obtains supplies, organizes snacks for classes.
Presents program information in various internal and external community meetings and functions as requested.
Performs other duties as assigned.
Qualifications:
High School Diploma or General Education Diploma (GED).
One year of administrative support and customer service experience.
Two years' administrative support experience in a social services or healthcare setting is preferred.
Applicable license or certification.
Some positions may require program specific certification.
Valid Driver's License and proof of automobile liability insurance coverage.
Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 9 on the language proficiency scale to receive bilingual differential pay.

Position Summary:
The Health Informatics Specialist plays a key role in bridging technology and clinical care by supporting, implementing, and optimizing Electronic Medical Record (EMR) systems and other core health applications for the Cowlitz Indian Tribes Health and Human Services (HHS) Department. This position is responsible for working directly with clinicians, providers, and IT staff to ensure that health information systems effectively support patient care, clinical workflows, and regulatory compliance, including HIPAA.
We are seeking a candidate with hands-on experience leading or supporting EMR implementations, as well as a deep understanding of clinical environments. The ideal candidate will be comfortable collaborating with healthcare professionals to translate clinical needs into system functionality, providing training and support, and continuously improving system use to enhance care delivery. This position requires both technical knowledge and strong interpersonal skills to successfully align health IT systems with the realities of clinical practice.

* Experience within a Windows Server environment preferred.
* A combination of post-high-school education and experience serving in a similar position may be used to meet the education and experience requirements. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities.
