Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.

Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.

The staff member must be able to assist with the orientation process of new division personnel. The staff member is expected to attend and participates in unit council meetings. The Behavioral Wellness Center at Girard, in partnership with the Be Well at MPAC initiative, leads The Philadelphia Model: Addressing Serious Mental Illness (SMI) in Long-Term Care. Located at 4001 Ford Road in Philadelphia, this structured therapeutic residential program provides behavioral healthcare for older adults with serious mental illness who also require skilled nursing care.
We are currently hiring a Life Enrichment Specialist, also known as a Creative Arts Therapist, Recreation Therapist, or Activities Coordinator, to join our team. This role is 32 hours per week on the day shift, with every other weekend required. The Life Enrichment Specialist develops and delivers engaging group and individual therapeutic activities that promote emotional, cognitive, and social well-being for residents while contributing to interdisciplinary clinical care.
Responsibilities include:
Conducting life enrichment and activity assessments for new residents
Developing and leading group programs, creative arts therapies, recreational therapy sessions, and 1:1 therapeutic activities
Creating inclusive, seasonal, and culturally relevant programming
Supporting clinical care planning and contributing to MDS documentation
Maintaining accurate documentation including progress notes, attendance logs, and monthly reports
Coordinating therapeutic recreation programs, special events, and community outings
Collaborating with nursing and clinical staff to ensure continuity of care
Maintaining a safe, trauma-informed, and inclusive group environment
Responding to behavioral health needs, resident feedback, and clinical changes
Qualifications:
Bachelor's degree in Recreational Therapy, Creative Arts Therapy, Occupational Therapy, Activities Coordination, or a related field required
Certified Therapeutic Recreation Specialist (CTRS) or a master's degree preferred
Experience in long-term care, behavioral health, or working with older adults with mental illness required
Background in group facilitation, activities programming, or creative arts therapy preferred
Strong knowledge of recreation therapy principles, group dynamics, and trauma-informed practices
Proficiency in Microsoft Office and EHR documentation systems

The Mobile Therapist is responsible for case management and residential rehabilitation coverage which requires direct services to tenants who have long term mental illness to maintain residency in supportive housing. The heart of residential rehabilitative services lies in the ability to provide clinical individualized supports in a community setting that is based on each resident's strengths, interests and needs.
ESSENTIAL FUNCTIONS:
1. Assess tenants' abilities and mental health status, individualized needs and preferences and appropriateness of services from point of supportive housing occupancy from a clinical standpoint.
2. Assist tenants in developing and implementing a comprehensive individual service plan (Recovery Goal Plan) which will focus on Adult Daily Living Skills, money and medication management, meal planning, cooking, cleaning, tenant responsibilities and needed services.
3. Coordinate service planning to include members of tenants' treatment team and tenants' natural supports.
4. Conduct monthly reviews of service plans and modify as needed.
5. To perform home visits to tenants on assigned caseload at frequency clinical documented in-service plan.
6. Assist tenants with meeting the obligations of tenancy. (Must visit daily for first thirty (30 days) after occupancy in supported housing).
7. Supervise the administration of tenants' self medication protocols.
8. Develop an individual Crisis Intervention Plan for all tenants and provide crisis intervention services. Utilize clinical and emergency backup as needed.
9. Documentation of all tenant/ counselor activity in agency's online client registry (Credible) at time of contact.
10. Maintain entire chart of tenants' records, review and update tenant assessments, complete reports of services provided, services needed, and all other clinical and administrative records deemed necessary.
11. Maintain and update case records documenting progress towards recovery goals and objectives.

•Educates consumers on emergency procedures such as fire safety, natural disasters, loss of utilities, violence and abuse

**SUMMARY OF POSITION:**
Counsel and advise individuals and groups to promote optimum mental and emotional health, with an emphasis on prevention. May help individuals deal with a broad range of mental health issues. This position orients and trains new team members and provides day-to-day oversight as required.
**MINIMUM QUALIFICATION(S):**
+ Master's degree required.
+ One (1) year of experience required.
+ PA Act 31 Child Abuse Training Certificate.
