The Opportunity:
The Polysomnographic Specialist performs all aspects of care as outlined in national and departmental clinical standard of practice policy and procedure manual and in accordance with written verbal orders or approved protocol flow charts. This position will assist with MSLT and home sleep study set-ups. Clinical practice activities include but are not limited to the performance of diagnostic polysomnographic testing, assessment based therapeutic interventions and the analysis and scoring of polysomnographic records. The Polysomnographic Specialist accountabilities include the assessment and evaluation of histories and physicals, diagnostic, clinical and sleep related data pursuant to the development and monitoring of planned interventions in collaboration with the medical staff. The Polysomnographic Specialist supports and participates as appropriate in staff meetings, study quality, adherence to departmental protocols, continuing education, and professional growth development activities and performs other duties as assigned.

Responsibilities:
Under the supervision of the manager and the professional direction of the radiation oncologist, the radiation therapist performs various radiotherapy treatments and simulations at a technical level requiring minimal supervision of technical detail. The radiation therapist may be required to assist in the development of new techniques and processes for simulations, treatment, new equipment and new devices. After reviewing treatment prescription of the physician and set up instructions, the therapist delivers daily radiation therapy treatments, schedules patients for treatment and maintains accurate patient EMR. Participates in continuing education programs, departmental quality assurance program and maintains current licensure and accreditation in the Radiation Oncology profession.
Offers assistance to coworkers during free time.
Actively participates in department meetings.
Maintains flexibility and positive attitude throughout changing environment.
Is knowledgeable of equipment and techniques in special treatment procedures such as SRS/SRT, Deep Inspiration Breath Hold per department processes.
Utilizes imaging technologies for the explicit purpose of simulation, treatment planning and treatment delivery as prescribed by a radiation oncologist such as weekly ports, CBCT and IGRT.
Performs daily warm-up procedures in simulation and on linear accelerators.
Complies with department safety policies and procedures.
Provide compassion and emotional support to patients and their families.
Maintain a positive attitude towards patients and peers.

Home Care and Hospice Clinical Liaison

Saint Luke's Home Care & Hospice is seeking a Clinical Liaison who will work primarily on consults and referrals from SLHS hospitals facilitating patient transfer to SLHS Homecare and Hospice. The liaison will respond quickly to in hospital Hospice consults by meeting with patients and their families to educate on hospice services and facilitate a smooth and seamless transition to Saint Luke's Hospice House, Inpatient hospice or patient's home environment. The liaison will collaborate with Hospital and Hospice providers to determine Hospice eligibility and appropriate patient level of care upon discharge. The liaison may also consult on complex Saint Luke's Home Health referrals and Home Infusion teaches as needed.
Shift: Full-time, Monday - Friday, 8 a.m. to 5 p.m. with occasional flexibility for after-hour coverage.
The Work:
Visits patients (and their families) to discuss goals of care upon receipt of consult request.
Collaborates with Hospice provides on Hospice eligibility and appropriate level of care for patient.
Coordinates with Hospital providers and staff to provide a seamless transition for patients to SLHS Home Health, Hospice, and Infusion services.
Fosters partnerships and relationships, through day-to-day interaction with SLHS hospital staff, providers, and Care Progression team.
Participates in capacity management discussions and meetings as requested.
Provides support to intake and referral processes by documenting and creating episodes in EPIC.
Provide statistics and referral information to the Supervisor Clinical Referral Services upon request, always looking to improve the liaison role to meet the needs of the agency.

Licensure:American Registry of Radiologic Technologist, Nuclear Medicine, Ultrasound, MRI, CT, Mammography, or Interventional Radiology certification or equivalent and active registration in Imaging Services. State licensure may be required depending on the specific state of practice and modality of specialty.
Certifications:American Heart Association Healthcare Provider card or complete a course within 6 months in this position.
Preferred Qualifications:Education: Bachelor's degree.Experience: Leadership experience at the supervisory level or above.

Overview:

Qualifications:
Licensure: Preferred registration by the American Board of Registration of Electroencephalographic Technologists or at least successful completion of part I (the written exam) of the aforementioned exam and successful completion of part II (the oral exam) within two years but not required.
Experience: One year experience at the Technologist I level. Previous experience establishing good rapport with patient, public, and staff. Previous experience dealing with severely ill patients.
Certifications: Certified in CPR
Other: Independent judgment and initiative.
Preferred Licensure: Preferred registration by the American Board of Registration of Electroencephalographic Technologists or at least successful completion of part I (the written exam) of the aforementioned exam and successful completion of part II (the oral exam) within two years.

Role Purpose
The Graduate Nurse provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care after completing a nursing program and before licensure.
