Job Description:
1. Provide initial and ongoing assessment, evaluate effectiveness of PCP and progress made by person receiving services, participate in team meetings/case reviews with other providers and natural supports as needed and give input into person-centered plan modifications.
2. Identify opportunities for community connections and development/enhancement of natural support networks for people served. Work with service staff to educate people served on these options and access opportunities.
3. Use a range of communication skills and strategies to establish a mutual relationship with the individual, staff, co-workers, supervisors, other stakeholders and people who are important to the individuals receiving services.
4. Coordinate disciplinary actions with Regional Director or Clinical Director to ensure superior service for the individuals being supported.
5. Ensure requests for services are made considering eligibility, continued stay and discharge criteria for the service definition. Services and supports that match the person-centered plan and the individual's needs shall be in place and be fiscally viable.
6. Conduct clinical authorization and billing systems that ensure all aspects including, but not limited to: timeframes, audits, following through on clinically related issues preventing authorization approval or billing, how to resolve issues locally/regionally, conducting reviews of clinical documentation to ensure that it meets the requirements for the service being provided.
7. Prepare and submit required or requested reports, documents, assessments, evaluations and paperwork. Review and sign off on clinical documentation as required. Designated to release confidential information within assigned department(s).
8. Identify methods whereby services can be provided utilizing existing community resources whenever possible. Continuously use person-centered approaches and positive approach strategies when interacting with consumers. Integrate strategies into program plans.
9. Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
10. Demonstrate knowledge of emergency procedures including preventative and crisis services. Schedule and participate in on-call coverage.
11. Work with supervisor on the continual assessment and enhancement of services provided throughout the agency.
12. Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
13. Follow service definition guidelines for services being provided.
14. Complete all other relevant responsibilities as assigned by the supervisor.
15. Driving and travel may be required.

REM Iowa is seeking QIDP to provide oversight and support of individual programming and needs in assigned programs in the Cedar Rapids area. The QIDP will work closely with facility and day program staff to ensure consistency and ensure service design and delivery, providing each individual with an active appropriate treatment program. This position will observe individuals, review data and progress and revise programs based on individual needs and performance. The QIDP will also ensure that any discrepancies or conflicts between programmatic, medical, dietary, and vocational aspects of the individual's assessment and program are resolved and will ensure follow-up to recommendations for services, equipment, or programs.

Starting rate $20.75

KEY RESPONSIBLITIES:

* Write and update individual's POC, programs, data sheets, corresponding informed consents and staff training sheets; prepares for and participates in Plan of Care/ IDT team meetings
* Conducts training on individual programming for facility and day program staff
* Completes ongoing required documentation for each individual served, such as monthly documentation and progress notes.
* Ensures follow-up to recommendations for services, equipment, or programs; and ensures that adequate supports and assistive devices are present to promote independence.
* Conduct observations of all shifts at each facility and at the day program; provides feedback and recommendations to supervisors and staff regarding program implementation / observations
* Ensure Comprehensive Function Assessment's (CFA) are completed and reviewed annually; update as individual's needs change
* Assists with all legal, licensing, regulatory, and certifications activities as needed. Monitors plan of correction for ongoing compliance.
* Develops and maintains working relationships with all clients, families, funding sources and regulatory agencies, as appropriate, and monitors their satisfaction with services.

QUALIFICATIONS:

* Must be a QIDP (bachelor's degree in a human service related field and one year experience in direct service with people with developmental disabilities required to meet QIDP qualifications)
* Knowledge and understanding of ICF rules and regulations, preferred
* Skilled in written and oral communications
* An ability to build positive relationships at all levels within the organization; establish rapport quickly with individuals and staff alike
* Strong customer service orientation - established level of patience, attention to detail, and strong follow through

Community Support Team (CST) Qualified Professional

Easterseals UCP (ESUCP) seeks a Qualified Professional to provide a full range of treatment functions for clients. The Community Support Team (CST) in Raleigh will treat clients with mental health or substance use disorder diagnoses that meet the needs of two life domains described in the CST service definition.


Responsibilities include and are not limited to:


  • Coordinates services and assures person-centeredness in all aspects of the service planning process.
  • Develops holistic, integrated interventions.
  • Participates in the initial development, implementation, and ongoing revisions of the Individuals' Person-Centered Plan (PCP), Crisis Plan, and Housing Support Crisis Plan. Also assists with the submission of periodic authorization requests for continuation or establishment of services to the Local Management Entities - Managed Care Organizations (LME-MCO).
  • Participates in "first responder" coverage and crisis response with the LME-MCO, and provides crisis support to the individual by the initiation of Wellness Recovery Action Plan (WRAP) or other crisis support plans. When required, CST must make immediate telephone contact with the individual, and face-to-face contact within 90-minutes after the crisis has occurred.
  • Participate in the provision of structured rehabilitative interventions to individuals served, such as:
    • Assessment, Individual Housing, and Tenancy Sustaining Services
    • Money Management and Entitlements
    • Activities of Daily Living
    • Personal Health, Wellness, and Recovery
    • Promote Community Integration



Requirements:


  • Bachelor's degree from approved, accredited institution
  • Ability to be credentialed as a Mental Health Qualified Professional or Associate Professional according to 10A NCAC 27G.0104
  • Prefer Certified Peer Support Specialist!
  • Knowledge of community housing resources
  • Possess the knowledge, skills, and abilities to successfully provide CST to adults with mental illness and substance abuse disorders
  • Minimum of two years' experience working with the Adult Mental Health Population
  • Possess current, unrestricted, state appropriate driver's license
  • Possess current, unrestricted, vehicle insurance and provide own transportation for use in fulfilling job requirements
