What Does A Quality Improvement Coordinator Do At Rochester Regional Health

* Maintains knowledge regarding the most current regulations related to MDS time frames, question content and submission requirements.
* Sets MDS time lines for each new admission, quarterly and annual reviews, significant change reviews.
* Communicates this information in written format to all departments.
* Reviews each MDS for accuracy by consulting with hands on care givers and by reviewing the medical record.
* Use clinical knowledge to facilitate completion of MDS by all disciplines.
* Teaches MDS to new staff as requested.
* Communicates daily with the Director of Nursing regarding status of MDS’s.
* Communicates as needed with the billing department regarding MDS Submissions.
* Monitors admission PRI’s completion, assigning PRIs to Nursing department staff as necessary.
* Determines Medicare eligibility for all new admissions and communicates to appropriate departments

What Does A Quality Improvement Coordinator Do At Lancaster General Health

* Coordinates, evaluates, and monitors the implementation of the company’s continuous Performance Improvement Plan utilizing accepted PI principles, tools and measurement techniques.
* Prepares and/or coordinates statistical reports on the results of utilization reviews, clinical record audits, Medicare claim denials, customer satisfaction surveys, and the Quality Improvement Plan.
* Directs utilization review and client medical record audits according to the company’s policies.
* Prepares and/or coordinates statistical reports on the results of utilization reviews, client medical record audits, Medicare claim denials, customer satisfaction surveys, and the Performance Improvement Plan.
* Monitors and coordinates the implementation of corrective action plan for deficiencies identified through utilization review, clinical record audit, Medicare claim denials, client satisfaction surveys, accrediting body and/or State surveys, and the Performance Improvement Plan.
* Serves as a resource on regulations for certification, licensure, accrediting body standards and Medicare eligibility requirements.
* Evaluates the need for and plans special classes, conferences, and meetings to improve level of staff performance and quality of care.
* Is responsible for the oversight of the clinical compliance review activities to assure the organization is operating within state and federal regulations.
* Coordinates internal clinical audits and reports on audit results.
* SECONDARY FUNCTIONS:** **The following duties are considered secondary to the primary duties listed above:
* Participates in the development and implementation of standardized policies and operating procedures.
* Participates in the development of critical pathways and clinical data to be utilized for standardized outcome management.
* Implements internal review practices to ensure areas of clinical risk are assessed using the OIG work plan as a reference for focusing review efforts.
* Coordinates yearly external compliance reviews, as directed by the corporate compliance officer/President or Board of Directors.
* Other duties as assigned

What Does A Quality Improvement Coordinator Do At SUNY Upstate Medical University

* review of reportable events based on standards set by the NYS Department of Health and DNV.
* Major focus for this position will be the management of medical malpractice case reviews, conducting and documenting quality of care reviews; interface with Physician Advisors and trial Legal Council; work in conjunction with Patient Relations; Department of Health onsite visits.
* Coordinates meetings between Legal and Trial Council with State employees.
* Identifies opportunities for quality improvement initiatives.
* Other duties will include; facilitating root cause analysis of assigned cases, utilizing the approved format.
* Evaluate assigned mortality reviews.
* Conducts failure and effect mode analysis - (FMEA's) as assigned by Director.
* Performs chart assessment and review of patient care evaluation studies as assigned for internal and external quality reviews (DOH Complaints; OPD Licensed Health Care Professional Reporting).
* Coordinates appropriate benchmarking projects.
* Coordinates data abstraction and submission as appropriate.
* Represents Risk Management, at committees and councils as assigned.
* Participates in quality assurance and risk activities in other areas as assigned.
* Applies time management skills to ensure duties are completed as per policy and protocol.
* NYS licensed/registered nurse (or eligible) required.
* Bachelor's Degree in Nursing (or healthcare related field) and three years RN acute care experience OR Associates Degree and 5 years RN acute care experience required.
* Excellent communication and customer service skills required for interface with health care providers, regulatory agencies and other health care related entities.
* Candidates with RN ICU experience, quality review experience and/or familiarity with DOH Regulatory reporting requirements preferred.
* CPR certification required.
* Competent in Microsoft Office products; clinically competent, excellent communication skills both verbally and written, excellent customer service

What Does A Quality Improvement Coordinator Do At NYU Medical Center

* Facilitates improvement initiatives, which includes but not limited to, monitoring trends in metrics, works collaboratively with physicians, administrative leadership, professional and paraprofessional staff in rapid cycle PDSA’s.
* Participates in assigned standing committees and improvement teams.
* Acts as “Team Leader” or “Facilitator”, for improvement teams.
* Analyzes data to identify trends and opportunities for improvement relating to HEDIS measures and CMS scores.
* Prepares data into meaningful information using reports, graphs, tables, slides and other documents to communicate results.
* Collaborates with the Information Technology team to streamline and use technology, ie automation to facilitate the overall quality improvement activities, which would include data collection, metrics development, publication of metrics and or improvement results.
* In collaboration with the Director of Risk, investigates reported and suspected deviations in patient care outcomes and “near miss” situations, facilitates appropriate review by physician, and/or other professional staff, as appropriate.
* Completes the investigation and review process of such deviations according to policy
* Communicate investigation findings, track issues to resolution and recommend improvement protocols to prevent further similar incidents and or correct adverse patterns/trends where found.
* When appropriate completing/revising configurations in the electronic medical record (identified through event investigations), specifically improving documentation, simultaneously enhancing the network’s risk reduction program.
* Educates and acts as a resource to all personnel on quality methodologies and regulatory standards related to quality and patient safety.
* Assists in the integration of PCMH standards and guidelines into the overall Quality Improvement Plan.
* Collaborate with the appropriate clinical and administrative leadership in developing/revising policies and procedures.
* Execute other tasks as assigned
