The Nurse Residency Program for new graduates offers:
- A Fulltime, benefit eligible nursing position.- A 12-month program to smooth transition into practice.- A series of thoughtfully developed classes that will enhance skills in critical thinking, assessment, leadership and clinical decision making.- Participation in this program is a part of the regular work assignment.- Classroom instruction specific to the clinical area using a focused case study approach to learning.- Clinical Experience under the guidance of an experience Preceptor.- Ongoing continuing education and professional development opportunities.- Dedicated nurse educators are available both during and following the program.
Essential Functions and Responsibilities: * Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.* Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.* Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.* Maintains accurate and timely documentation of clinical records and time and travel entries.* Integrates and coordinates the care of the at-risk population across the healthcare continuum.* Communicates clinical status and plans clearly, succinctly and accurately to interdisciplinary home care team, schedulers, team leaders, and physicians.* Serves as a patient advocate, educator and as central source of communication for the patient and their support systems, and referral sources.* Conferences with clinical management, physician and interdisciplinary team in order to provide high quality, cost effective care.* Develops/updates a plan of care appropriate to patient acuity/complexity and specific to the individual needs of the patient.* Care is outcome-oriented, reflects measurable progress and goals toward discharge.* Facilitates advance care planning discussions to ensure appropriate care coordination and high-quality care based on patient's long-term goals of care.* Delivers quality care consistent with Home Health Conditions of Participation, qualifying home care criteria, payer coverage criteria, and providing visits/treatments consistent with the plan of care.* Conducts clinical review of record to include multi-disciplinary review, coordination of care, provided in conjunction with visits and documentation.* Identifies risk for acute hospitalization and proactively prevents adverse events.* Demonstrates competency in comprehensive assessment/OASIS competency without oversight.* Maintains OASIS accuracy and integrity by reviewing all audits and reviews and making changes as necessary in a timely manner.* Responsible for accurate documentation (ie: care plan within required timelines, care and utilization of resources to achieve quality outcomes, conferences, discharges, billing, coding.)* Documentation reflects understanding and application of qualifying criteria for home care.* Reviews multidisciplinary care provision, conducts own clinical review and participates in peer audit, as applicable.* Assists in the promotion of services and products.* Performs “on-call” duties and weekend/after hour visits on a rotational basis as assigned, after orientation period is completed.* Maintains regular and consistent attendance at work.* Behaves in a manner consistent with Mission, Vision and Values..* Maintains compliance with OSHA, Accreditation Standards, and Risk Management guidelines.* Maintains compliance with Personnel policies and procedures.* Behaves in a manner consistent with all Corporate Compliance policies and procedures.* Performs other duties as requested by the Manager or Supervisor to facilitate smooth and effective operations of the organization.* Meets agency established benchmarks for quality and process measures, patient satisfaction, OASIS case mix, productivity and documentation timeliness.* Employee is responsible for professional communication with all members of the interdisciplinary team and home care operations. This includes but is not limited to responding to email, voicemail and telephony message; attending all required staff meetings; communicating with supervisor and other team members; reviewing all employee communication; and reviewing department newsletters.
Required Qualifications: - Graduate of State Board approved program for Registered Nurses.- WI RN license.- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.- Strong interpersonal skills.- Ability to work as a collaborative team member.- Ability to understand and apply guidelines, policies and procedures.- Strong computer skills.- Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.

Chart on his/her shift in accordance with facility policy, state guidelines and residents status.

Supervise residents who self-administer medication while following community and regulatory guidelines

POSITION SUMMARY
The Home Health Visit Nurse coordinates and directs care based on individual patient need. This position is responsible for providing care for the Home Health patient population requiring the use of advanced assessment, teaching, and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, and appropriate documentation is completed. The Nurse provides support by completing necessary patient visits and home health admissions when appropriate. They are responsible for evaluating the patient's appropriateness of home health and developing the home health plan in conjunction with the physician. They educate patients, families, caregivers, and community providers to safely deliver needed health services. The visit nurse works with the RN Case Manager to plan and implement patient services and ensure these services are effective in maintaining patient wellbeing and safety.
TECHNICAL RESPONSIBILITIES
Performs comprehensive assessments of physical, functional, psychosocial, and cognitive status of the home health patient utilizing interview observations and physical exam techniques.
Assesses the home environment for safety, infection control, and community resource needs.
Plans with the patient and family for care which is feasible within the physical, financial, and emotional resources of the family.
Establishes individualized, measurable goals in consultation with the patient, family, and other health care providers.
Anticipates home health needs and seeks a wide range of community resources to facilitate problem solving.

Sets priorities of home health visits, adapting to the changing needs of the home care patients and families.

Facilitates and participates in formal and informal interdisciplinary care conferences to address the needs of complex patients.
Always maintains an updated EHR for each patient and meets required regulatory deadlines.
Participates in and assists in the ongoing development of home health policies, procedures, standards of care and documentation systems.
Initiates interdisciplinary collaboration to positively impact the outcomes of health care provided to patients and their families in the community.
Involves the patient and family in the plan of care, incorporating their cultural, spiritual, and other belief systems.
Assists leadership in identifying potential clinical practice issues and contributes to the development of specific plans to improve home health patient outcomes.
Demonstrates knowledge of research findings related to home health nursing by implementing current practice standards.
Incorporates changes in clinical practice to reflect new advances and ensure effectiveness of planned interventions.
Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
Quality: Focuses on improving processes and champions efficiency in daily work activities.
Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization's occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).

JOB REQUIREMENTS
Licenses:
State of Wisconsin Registered Nurse license
State of Wisconsin Valid driver's license
Education:
Bachelor's degree in Registered Nursing preferred.
Experience:
Minimum of one year of recent experience in nursing
Experience and strong knowledge base in Home Health preferred.
Miscellaneous:
CPR/BLS certification
Current IV therapy skills
Experience with wound/ostomy care
