The Social Worker provides services in the dialysis unit to patients and their families to support and maximize the social functioning and adjustment of the patient with End Stage Renal Disease.
Key Tasks Include:
* Conducts initial patient psychosocial assessment and comprehensive plan of care
* Provides support, information and assistance in treatment planning
* Develops and continues to monitor psychosocial goals for plan of care
* Counsels new patients and families on pre-dialysis education, including treatment options
* Acts as advocate on behalf of patient and family
* Completes and documents initial and ongoing comprehensive assessment, evaluations and quarterly notes, along with documenting and monitoring ongoing psychosocial issues and patient goals
* Facilitates and/or refers patients and families to support/education groups
* Facilitates education on Advance Directives and treatment modalities
* Coordinates and participates in patient/family care conferences with interdisciplinary team
* Analyzes and provides information and facilitates referrals to community, private, state and federal agencies based on psychosocial needs
* Provides crisis intervention expertise as necessary
* Provides assistance to patient and family in adjusting to dialysis and altered lifestyle, and provide resource information
* Arranges patient travel arrangements for both guest and permanent patients and ensure completion of arrangements
* Coordinates patient transfers/ visit to other facilities
* Provides assistance to patient regarding health insurance
* Maintains patient confidentiality and administers patient care in a respectful manner
Position Requirements:
* Licensed Social Worker in New York State; LMSW required; LCSW a plus
* At least two (2) years of prior work experience as a Social Worker
* Prior experience as a Dialysis Social Worker or working with dialysis patients
* Strong interviewing skills for patient evaluation, problem solving, and recording
* Possess strong analytical skills to assist patients in problem solving and resolution
* Capable of communicating clearly and effectively with all types of physically and mentally challenged patients and their families

Job Requirements Our Registered Nurses provide high quality patient care to patients suffering from the most advanced stage of chronic kidney disease, known as End Stage Renal Disease (ESRD). Nurses are on the front line of patient care, improving the quality of life of every patient. This role is all about the relationships and bond you will create with patients and their families.
Work Experience
* Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (BSN); three year diploma from accredited diploma program may be substituted for nursing degree.
* Registered Nurse License in the state of practice.
* Current BLS/CPR certification required.
* Meets all applicable state/licensing requirements.
* Previous dialysis, ICU, CCU, ER, or Med Surg nursing experience is preferred but not required.
