Job Summary: SWCM I: Within the Case Management Department this position provides social work and discharge planning in predictable situations using established policies, procedures, ACMA guidelines, and NASW code of ethics. This position requires direct to regular supervision, but is able to recognize attributes (i.e., isolated traits, objective measures or tasks) of a social work situation without situational experience. This position may participate in other hospital committees and teams as appropriate. Incumbents working at this level are generally assigned to one area of the hospital. This is the trainee/entry level with direct supervision from an experienced Case Manager. Assignments are meant to orient the incumbent to BRH Case Management services, protocols, techniques, and allow for on-the-job training in preparation to advance to the journey level. Collaboration with partners along with frequent communication is essential. At this level, incumbents are experienced and hold a Master's Degree in a relevant field giving them the ability to independently plan and implement social work care for the most complex cases, provide mentoring of SWCM I's & II's, and assigned to float to other locations in the hospital if needed. Positions at this level also conduct clinical evaluations and assessments.
