The Health Records Clerk works closely with clinic, scheduling and intake departments. Also collaborates daily with the interdisciplinary teams and outside providers for efficient coordination of care of our participants. The Health Records Clerk also responds to requests for medical records and performs clerical duties. MUST LIVE IN ANAHEIM, BAKERSFIELD, OR FRESNO, CA AREA Essential Job Functions Duties include, but are not limited to: Prepares charts for new participants in collaboration with intake department. Ensures that all past medical history is received and ready for review by initial assessment. Scans documents into participants record daily. Maintains participant records by collecting; test results, specialty consults, procedure results, radiology results, dialysis flowsheets, skill nursing summary's and hospital summary's, and other relevant medical data. Collaborates with the interdisciplinary team in participant care planning. Monitors outside scheduled appointments daily, collects appointment information and updates the specialty order accordingly. Routes notes to ordering provider for review and care coordination, Review and maintains the scheduled orders in queue by providing follow-up and documenting updates. Adhering to professional standards, IIH policies and procedures, federal, state, and local requirements. Enhance health records credibility and the reputation of IIH by accepting ownership for accomplishing new and different requests. Provide recommendations for opportunities of growth in the health records department. Attend and participate in staff meetings, in-services, projects, and committees as assigned. Adhere to and support the center's practices, procedures, and policies including assigned break times and attendance. Accept assigned duties in a cooperative manner; and perform all other related duties as assigned. Be flexible in schedule of hours worked. May require use of personal vehicle. Qualifications Knowledge, Skills and Abilities Proficient knowledge of computer skills. MS Office (Word, Excel, Access, PowerPoint, and Outlook). Knowledge of electronic health records systems. Knowledge of general office procedures, equipment and filing systems. Ability to communicate effectively, both orally and in writing. Ability to quickly learn department policies, procedures, goals, and services. Skill: Attention to detail and accuracy. Ability to change priorities regularly. Ability to cope with mental and emotional stress related to the position. Function independently, have flexibility, and personal integrity. Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads. Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment. Experience Minimum of one (1) year experience in Health Records required Minimum of one (1) year of documented experience working with a frail or elderly population required. Education and Certification High school graduate or equivalent. Medical Assistant Certification preferred. Medical Terminology certificate preferred Current Driver's License in the State of California Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
