Primary Responsibilities The Activity Director will be responsible for planning, coordinating, and directing the resident's activity program and the maintenance of necessary documentation. Essential Functions * Organize both individual and group activities based on the needs of the residents * Ensure that multiple activities are occurring for both high and low functioning residents * Complete section E of the Minimum Data Set (MDS) timely and accurately * Develop the activities component of the Comprehensive Care Plan from the completed activity assessment * Complete an annual assessment for each resident on the activities component of the Resident Assessment * Provide activities for residents that are bedfast and/or unable to participate in group activities (one to one) and documents in the appropriate record * Prepare monthly calendar and post in an area that is accessible and highly visible and frequented by residents and visitors. The primary purpose of your job position is to perform the day-to-day activities of the Laundry Department in accordance with current federal, state, and local standards, guidelines, and regulations governing our facility, and as may be directed by the Administrator and/or the Housekeeping Supervisor, to assure that our facility is maintained in a clean, safe, and sanitary manner, and that an adequate supply of linen is on hand at all times to meet the needs of the residents. Ensure that work/cleaning schedules are followed as closely as practical. Report all accidents/incidents to your supervisor. Coordinate daily laundry services with nursing services when performing routine assignments in resident living areas. Follow established fire safety policies and procedures. Wear and/or use safety equipment and supplies (e.g., back brace, mechanical lifts, etc.) when lifting or moving heavy objects. Ensure that assigned work areas are maintained in a clean, safe, and sanitary manner. (e.g., dryer lint traps) Follow proper techniques when mixing chemicals, disinfectants, and solutions used in the work area. Refer to material safety data sheets (MSDSs) when necessary. Use appropriate personal protective equipment and supplies when handling infectious materials. Keep supervisor informed of supply needs. Keep work area(s) free of hazardous conditions, i.e., spills, excess laundry carts, baskets, supplies, equipment, etc. Clean work/supply carts, equipment, etc., as necessary/directed. Ensure that equipment is cleaned and properly stored at the end of the shift. Perform assigned tasks in accordance with established laundry procedures. During emergency conditions, assure that clean laundry, linen, garments, etc., are distributed to designated areas as instructed. Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

The Hospitality Aide will provide assigned residents with non-clinical care and services in accordance with directives given by the Charge Nurse. Works under the general direction and supervision of the Charge Nurse. 1. Follow resident assignment schedule as made out by charge nurse. This requires the ability to be mentally alert and communicate verbally and in writing information regarding resident's condition and care. 2. Walk with ambulatory residents or push in a wheelchair as required. Primary Responsibilities The Social Worker will be responsible to work with the residents in overseeing and assisting in the admission process, identification of needs and problems, make referrals to community resources, and ensuring the mental and social health of each resident. Primary Responsibilities The NP is responsible for practicing the nursing profession ethically, dutifully, and according to and within the Scope and Standards of Practice of the Texas Board of Nursing. The NP is responsible for Provider-level care coordination, as outlined in the Texas Independence Health Plan (ISNP) Model of Care, for all ISNP Members. The NP is generally expected to be in his/her facilities during customary working hours (approximately 8:00am - 5:00pm) to ensure that these facilities have adequate Provider-Level coverage to meet patient/resident/member health care needs while also allowing the NP to have adequate time for documentation, and rotational weekday/weekend on-call responsibility. Requires the ability to lift at least 25 pounds and remove assistive devices to clean properly. 15. Safely utilize equipment. Report any equipment needing service or repair immediately. 16. Participate in emergency fire or disaster drills and other in-service training as scheduled. Must be able to safely ascend and descend stairwells and assist in an evacuation. 17. Follow Universal Safety Precautions in order to protect self, residents and other staff members. Rinse soiled linen in preparation to be sent to the laundry this involves the capability to withstand strong foul odors and the ability to tolerate wearing necessary personal protective equipment. 18. Bring laundry barrels to laundry room; get linens for beds, makes unoccupied beds. 19. Bring residents to and from daily activities. 20. Assist with actives going on in building. 21. Clerical functions. 22. Must be capable of maintaining regular attendance in accordance with facility attendance policies. Perform special duties as assigned. 23. Promotes positive public relations with patients, residents, family members and guests. 24. May be required to perform other duties and tasks as assigned by the Administrator, Director of Nursing and Charge Nurse in accordance with the qualifications and training provided.

The RN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care. Essential Functions During rounds observes, assess, documents, and reports resident condition/changes Assures implementation of care plans as directed Administers medications as prescribed Conduct rounds with physician and record visits in clinical records Perform narcotic reconciliation with incoming and outgoing shifts Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures Oversees and monitors aides Initiate and complete Incident and Accident Reports per facility policy Discharge residents per facility policy Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition Communicate with resident's point of contact when they ask for a status update or if there is a change in condition Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator

Primary Responsibilities The Dietary Aide follows the direction of the Certified Dietary Manager and state guidelines to ensure that food is properly prepared, served to residents, and after meals are completed cleanup of the dining hall and dishes. Essential Functions * Assist with food prep and cooking following universal precautions and state/federal regulations * Preparing and serving beverages * Setting up and passing trays for meal service * Stocking snack cart * Bussing of tables and trays * Cleaning the kitchen work area * Dishwashing * Follow proper handwashing guidelines * Additional special projects or tasks can be assigned at times by the CDM or the Executive Director
