The Central Admissions Specialist supports the admissions department's activities in the pre-admission, admission, discharge, and follow up processes. Primary Responsibilities The Medical Records Coordinator will maintain and update the residents' clinical records in a confidential, systematic, well-organized manner in conjunction with acceptable policies/procedures that include OBRA guidelines; Texas State Standards and Individual Nursing Home Standards. Ensure that assigned work areas are maintained in a clean, safe, and sanitary manner. (e.g., dryer lint traps) Follow proper techniques when mixing chemicals, disinfectants, and solutions used in the work area. Refer to material safety data sheets (MSDSs) when necessary. Use appropriate personal protective equipment and supplies when handling infectious materials. Keep supervisor informed of supply needs. Keep work area(s) free of hazardous conditions, i.e., spills, excess laundry carts, baskets, supplies, equipment, etc. Clean work/supply carts, equipment, etc., as necessary/directed. Works under the general direction and supervision of the Charge Nurse. 1. Follow resident assignment schedule as made out by charge nurse. Walk with ambulatory residents or push in a wheelchair as required. Review other consultant reports to promote consistency in the application of rules and regulations. Review any and all regulatory reports (i.e. 2567, Quality Monitoring reports, Quality Indicators, etc.). Review policies and procedures adopted by the facility to promote regulatory compliance. Review facility reports (i.e., 24-hour nursing report, facility QA reports, staffing hours, etc.) and evaluate compliance with TDADS regulations. Actively participate in corporate quality assurance. Recommend policy and procedure changes and/or updates. Attend quarterly Policy/Forms/QA Committee meeting and actively participate in the development of facility policies and procedures. The essential functions for the AR Analyst are to bill required claims within manner, filling, and ensuring that payments are received. Each AR Analyst has a monthly goal to meet. The AR Analyst is responsible for: * Responsible for effective communication from central billing office to field staff and facility staff to facilitate prompt and clean claims submission for facilities assigned. * Responsible for census for each assigned facility. This includes being familiar with payer contracts, and checking census regularly to ensure revenue and residents are booked and being billed per our contracts and payer trees are set up correctly. * Responsible for Billing all claims monthly by deadlines set forth depending on payer. * Responsible for working claims rejections and denials in a timely manner. * Responsible for held claims daily review and follow up as well as documentation until claims release. * Responsible for claims follow up until payment is received on all claims for assigned facilities. * Responsible for daily cash posting of deposits within 24 hours of receipt and reconciling check registers to verify all payments posted for the month. * Responsible for Aging review and clean up for assigned facilities. This includes reclassing and census line corrections as identified during such reviews. * Responsible for notifying ADR team regarding denials or additional documentation requests as identified for medical necessity or medical records requests. Claim corrections due to coding issues will be handled by the assigned biller. * Responsible for billing co-insurance claims after receipt of primary payer payment. * Responsible for reporting issues with specific payers to CBO AR Manager so that complaints are addressed timely and with the appropriate State departments should the need arise. * Responsible for keeping accurate and up to date billing notes in PCC for all accounts worked. * Responsible for Bad Debt completion on receivables each month. * Maintaining company collection goals and provide reasons for not meeting collections. * Participate in monthly aging reviews with Accounts Receivable Manager and Supervisor. * Ability to maintain proper time and attendance along with possessing a positive attitude and productive work environment. * Any other assigned functions as needed by the department as deemed necessary by CBO AR Manager.

JOB DUTIES/ KNOWLEDGE: * Conducts assessments and reassessments of the patient's communication and swallowing disorders by applying diagnostic and prognostic procedures. * Plans, implements, and supervises an individualized therapeutic program designed to restore and/ or improve communicative activities of daily living, and to treat swallowing disorders. * Coordinates and collaborates with the Rehab Director, or designed professional, to ensure integration of the therapeutic program in the plan of care. * Guides and teaches the patient in techniques, exercises, or precautions designed to improve the patient's ability to swallow and communicate. * Observes, records, and reports the patient's reaction to treatment and any changes in the patient's condition or treatment. Documents the extent of the patient's involvement in the decision affecting his/her care. * Instructs other agency personnel and patients family members in activities of speech therapy in which they may safely and effectively assist the patient. * Participates in patient case conferences to coordinate the patient's plan of care and promote positive therapeutic outcome. * Participates in continuous assessment and evaluation of the agency via Quarterly Record Review, or the quality Assurance/ Improvement Program. * Fallows state guidelines and maintains appropriate documentation when supervising an Assistant in speech. * Maintain code of ethics set by the American Speech Language Hearing Association. PHYSICAL REQUIREMENTS Sight and hearing sufficient to comprehend written and verbal communication, ability to work a minimum of eight (8) hours per day while driving, standing, sitting, and completing reports and documentation.

The primary purpose of your job position is to perform the day-to-day activities of the Laundry Department in accordance with current federal, state, and local standards, guidelines, and regulations governing our facility, and as may be directed by the Administrator and/or the Housekeeping Supervisor, to assure that our facility is maintained in a clean, safe, and sanitary manner, and that an adequate supply of linen is on hand at all times to meet the needs of the residents. Ensure that work/cleaning schedules are followed as closely as practical. Report all accidents/incidents to your supervisor. Coordinate daily laundry services with nursing services when performing routine assignments in resident living areas. Follow established fire safety policies and procedures. Wear and/or use safety equipment and supplies (e.g., back brace, mechanical lifts, etc.) when lifting or moving heavy objects. Ensure that assigned work areas are maintained in a clean, safe, and sanitary manner. (e.g., dryer lint traps) Follow proper techniques when mixing chemicals, disinfectants, and solutions used in the work area. Refer to material safety data sheets (MSDSs) when necessary. Use appropriate personal protective equipment and supplies when handling infectious materials. Keep supervisor informed of supply needs. Keep work area(s) free of hazardous conditions, i.e., spills, excess laundry carts, baskets, supplies, equipment, etc. Clean work/supply carts, equipment, etc., as necessary/directed. Ensure that equipment is cleaned and properly stored at the end of the shift. Perform assigned tasks in accordance with established laundry procedures. During emergency conditions, assure that clean laundry, linen, garments, etc., are distributed to designated areas as instructed. Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

Primary Responsibilities The Certified Mediation Aide will prepare, administer, and document prescribed medications per protocol Essential Functions * Administers prescribed medications and treatments as defined by state regulations in accordance with company policy and procedure. * Observe and report to the facility's charge nurse reactions and side-effects to medication shown by a resident * Take and record vital signs prior to the administration of medication which could affect or change the vital signs * Reports to charge nurse and documents reasons prescribed drugs are not administered. * Administer regularly prescribed medication which the permit holder has been trained to administer only after personally preparing (setting up) the medication to be administered * The medication aide will document the administered medication in the resident's medication administration record * Administer oxygen per nasal canula or a non-sealing face mask only in an emergency; immediately after the emergency, the medication aide shall verbally notify the licensed nurse on duty or on call and appropriately document the action and notification. * Apply specifically ordered ophthalmic, OTIC, nasal, and vaginal medication * Be able to accurately follow all principals of good medication administration * Assist in customary daily requirements and tasks in care and treatment such as bathing, feeding, dressing, observing intake of food, care of hair, nails, moving residents from area to area, responding to calls, etc., which includes lifting, transferring * Helps charge nurses with the cleanliness of the nursing station, medication room and supply room.
