Works with case management and social work to identify available resources for formula supplies between WIC, insurance, and community programs. Documents need for each resource, how much will come from each resource, and assists with document completion for insurance, DME and community resource formula coverages. Identifies goals and provides age-specific, culturally appropriate nutrition counseling and education to patients and their families/caregivers throughout the lifecycle. The Multi-Facility Registered Dietitian functions as the Facility Consultant Dietitian as directed by the Executive Director. This position will be based in Winnfield and will also serve Natchitoches Nursing Rehabilitation Center and Winnfield Nursing and Rehabilitation Center. Travel is required. Completes all nutritional paperwork for new admissions, visits new residents for diet history, reviews medical record for pertinent nutritional information, records necessary information, notifies dietary of likes / dislikes, beverage preferences, any food allergies, and diabetic meal pattern if needed. 5. Attends and conducts periodic meeting/workshop training for the Food Service Director and Facility as needed. 6. Assists Executive Director in forecasting dietary capital expenditures. 7. Assists the facility Executive Director when requested in hiring and training of facility Food Service Director. QUALIFICATIONS SUMMARY OF QUALIFICATIONS: 1. Possesses a valid and current registration for the American Dietetic Association and license or certification as mandated by the state where employed. 2.
