Assess the nutrition status of assigned patients and implement all components of the nutrition care process, according to food and nutrition department policies and local regulatory standards (IDPH, TJC, CMS, etc.). Monitors and evaluates nutrition interventions, makes appropriate adjustments to plans as needed.
* Assess, interpret, plan, monitor, and document the nutritional status and nutritional needs of patients using evidence-based standards of care, policies and practice guidelines. Perform a nutrition-focused physical exam on patients. Identify and provide age-specific nutrition care and meet the cultural needs of the patients and their families. Work within an inter-disciplinary team to provide a plan of care to achieve the patient's health goals.
* Educate patients, care givers, students, and other health care colleagues on the effect of therapeutic diets and nutrition support to maintain/improve nutritional status.
* When assigned, provide support and supervision to the daily infant formula preparation routine which includes identifying and calculating recipes to individualize the formula to meet nutritional needs of the infant.
* Participate in department activities to enhance patient satisfaction.
* Keep current on nutrition research and incorporate into daily practice. Other duties as assigned.

* Specify Degree(s): Dietetics, Nutrition or Family & Consumer Science with ACEND (Accreditation Council for Education in Nutrition and Dietetics) requirements completed.

Enters nutrition-related orders, with confirmation by the Licensed Independent Practitioner (LIP), such as therapeutic diets, nutrition supplements, and enteral nutrition.
Provides parenteral nutrition recommendations for ordering by LIP. Reviews LIP orders as appropriate and recommends orders for enteral and parenteral feeding regimens as necessary.

Role Description
The Registered Dietitian reports to the Director of Dietetics & Nutrition Services at Skyway Behavior Health and is responsible for providing nutrition therapy and groups to patients with a specific focus on co-occurring eating disorders. Essential Functions/Core Responsibilities
Responsible for initial nutritional assessments, individualized treatment planning for all patients and the development of meal plans if needed for clients with co-occurring eating disorders/allergies or medical conditions
Conducts weekly nutrition groups (planning and implementation) for all patients at Skyway including experiential groups
Conducts individual sessions with patients based on individualized dietary needs
Responsible for managing and coordinating food orders with outside catering vendors
Developing individualized meal plans based on MD orders
Promote quality patient services and overall patient satisfaction. Responsible for resolution of any patient or customer concerns regarding nutrition team.
In coordination with the leadership team, responsible for overseeing clinic operations and programs to achieve established strategies, goals and objectives aligned with Joint Commission Standards elevated to the nutrition department, safe storage, and food preparation.
Establishes and maintains effective communications and working relationships with physicians, providers, and all colleagues. Responds in a timely fashion to provider concerns and resolves problems as appropriate. Actively leads and participates in established committees as needed.

The Dietitian provides nutritional services to patients in order to maximize the patients' well-being and improve outcomes. Works as part of an Inter-Disciplinary Team to maximize patient care and support the company to meet the goals of the QAPI activities.
PRIMARY DUTIES / ACTIVITIES:
Monitor subjective and objective data to determine the need for timely intervention and follow-up.
Review patient lab work and make individualized recommendations/adjustments to diet and bone medications.
Recommend therapeutic diets in consideration of cultural preferences and changes in treatment based on the patient's nutritional needs in consultation with the patient's physician.
Counsel a patient, patient's family, and a patient's significant other on prescribed diets and monitoring adherence and response to diet therapy.
Referring patients for assistance with nutrition resources such as financial assistance, community resources or in-home assistance.
Follow federal and state regulations.
Collect objective and subjective patient data to assess nutrition status. Responsible for a comprehensive nutrition assessment with an educational component within 30 days from patient admission to dialysis facility
Conduct a nutrition reassessment annually or more often if indicated.
Participate in monthly clinics with the patient and family members to review the prescribed diet and dietary compliance and make adjustments to bone medications per protocol and in conjunction with the Physician.
Available to answer questions and discuss problems with the dialysis patients when excessive weight-gain occurs, or when other instances of suspected dietary non-compliance occurs.
Available for staff education regarding the nutritional care of the patient.
Participate in monthly interdisciplinary patient care conferences.
Assure that follow-up progress notes are written in each patient's records each month.
Perform regularly-scheduled evaluations of standard menus as necessary for renal patients, revising and/or updating menus as necessary, and participates in monthly interdisciplinary patient care conference.
Document patient's care plan and long-term program.
Will be available for conferences with salesmen concerning products that could be used in providing nutritional care for renal patients and makes recommendations for possible purchases.

Position Summary:
Under the supervision of the Director of Food and Nutrition Services Department, the Nutrition Support Dietitian provides professional nutritional services that is appropriate for age and developmental care of adult and geriatric patients. While actual duties and responsibilities may vary, the following are characteristics of the work performed by the Nutrition Support Dietitian.
Responsibilities:
1. Exemplifies the RML REACH (Service Excellence) culture by demonstrating respect, excellence, appreciation, concern, and honor in all interactions with patients, families and co-workers, and as identified in departmental and hospital wide behavioral expectations.
2. Perform nutritional analysis that includes all sections of the Nutrition Care Process (NCP): assessment, diagnosis, intervention, monitoring and evaluation.

4. Order writing privileges allow the dietitian to be responsible for all oral diet enteral and parenteral nutrition orders.
5. Provide nutrition education for a variety of topics for patients, family, and staff.
6. Coordinate, perform and evaluate discharge planning.
7. Participates in medical rounds and other clinical-related meetings with the medical team.
8. Supervise and manage resources regarding the nutritional formulary.
9. Performs meal rounds and addresses complaints and or problems regarding diet orders, meals and feeding.

Role Description
The Registered Dietitian reports to the Director of Dietetics & Nutrition Services at Skyway Behavior Health and is responsible for providing nutrition therapy and groups to patients with a specific focus on co-occurring eating disorders. At Skyway Behavioral Health, we believe that learning to nourish yourself is a skill that empowers patients with both traumatic stress only and traumatic stress and eating disorder pathology.
Essential Functions/Core Responsibilities
Responsible for initial nutritional assessments, individualized treatment planning for all patients and the development of meal plans if needed for clients with co-occurring eating disorders/allergies or medical conditions
Conducts weekly nutrition groups (planning and implementation) for all patients at Skyway including experiential groups
Conducts individual sessions with patients based on individualized dietary needs
Responsible for managing and coordinating food orders with outside catering vendors
Developing individualized meal plans based on MD orders
Promote quality patient services and overall patient satisfaction. Responsible for resolution of any patient or customer concerns regarding nutrition team.
In coordination with the leadership team, responsible for overseeing clinic operations and programs to achieve established strategies, goals and objectives aligned with Joint Commission Standards elevated to the nutrition department, safe storage, and food preparation.
Establishes and maintains effective communications and working relationships with physicians, providers, and all colleagues. Responds in a timely fashion to provider concerns and resolves problems as appropriate. Actively leads and participates in established committees as needed.
Supports the organization's mission, vision, values, and strategic initiatives. Develops, implements, and improves processes to ensure that established policies, goals, and objectives are achieved.
