Position Overview:
The Dietitian will be responsible for completing comprehensive nutritional assessments, developing and implementing care plans, and collaborating with an interdisciplinary team to ensure optimal nutritional support for participants. This role involves both participant care and administrative responsibilities.
Key Responsibilities:
Participant Care - 60%
Develop and implement individualized nutritional Plans of Care and reassess every six months or as participant status changes.
Screen nutritional status, access to food, and coordinate services such as Meals on Wheels or grocery assistance.
Provide education and dietary counseling to participants and caregivers.
Monitor food intake, therapeutic diets, and recommend liquid supplements as needed.
Conduct home visits and communicate with caregivers and staff to ensure proper dietary management.
Maintain thorough and timely documentation in participant medical records.

About the Role
The Registered Dietitian (RD) is responsible for designing and implementing the nutrition services program working in collaboration with the interdisciplinary team (IDT). The Registered Dietitian shall acknowledge that patient/youth may include diabetics, renal, hypertensive, HIV positive, and acute/chronic illness.
+ Collect information on patient/youth nutritional history.
+ Participate in committees relating to nutrition, food service, bioethics, performance improvement/quality assurance and other issues.
+ Instruct institutional food service employees on the preparation and distribution of medical diets and provide dietary staff with alternative food product suggestions or allowed/acceptable food substitutions as needed.

Teamwork. You will collaborate with the Medical Director, physician/advanced practice provider, facility administrator, social worker, nurse, and patient care technician to maintain positive relationships with area hospitals, skilled nursing facilities, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes, with a focus on improving patient health and achieving target goals for patient outcomes. You will also help maintain a productive quality assurance program and participate in monthly Quality Assessment and Performance Improvement (QAPI) activities.
