The Registered Dietitian is an integral part of the interdisciplinary team, using nutrition education to help improve quality of life for our patients. RDs are responsible for assessing, monitoring and educating patients and their families on the importance of diet and nutrition in managing chronic kidney disease.
Schedule: Per-Diem, 10 hours/week, shifts start at 9am
Compensation: Pay range from $32-$40 per hour, depending on renal experience; experience preferred
Responsibilities
What You Can Expect:
Assist patients with making diet and nutrition choices that improve their quality of life
Complete comprehensive nutrition assessment for all new and at-risk patients
Collaborate with interdisciplinary team and patient to develop and implement individualized care plans based on nutritional assessment; review monthly and document progress toward established goals
Identify specific interventions necessary to meet nutritional needs based on evidence-based guidelines, best practices and current research
Educate patient and family/caregiver about renal diet and fluid management
Monitor monthly laboratory results and weight changes, make recommendations to meet established goals
Utilize a patient-centered care approach, respecting and responding to individual and cultural preferences while encouraging engagement
Participate in Quality Assessment Performance Improvement process, documenting and attending meetings per clinic policy
Compile and create nutrition materials to promote adherence to renal diet guidelines
Communicate nutrition concerns with care team, including attending physicians
Share kidney disease and nutrition advances with care team as needed

Required to travel to other WIC clinics, and to NYS DOH WIC and SCDHS WIC training locations.
Supervised by Nutritionist I and/or Nutritionist II/WIC Program Director. SCDHS WIC Administrative Office provides overall management, supervision and work assignments of 9 WIC clinics.

Completes required WIC Program and NYWIC related webinars and training modules to comply with NYS DOH WIC Program requirements..
Completes nutrition assessment, develops high-risk nutrition care plans and provides participant-centered nutrition counseling of WIC participants according to NYS DOH and federal WIC regulations.
Prescribes, tailors, authorizes, issues / re-issues WIC food packages and special formulas prescribed by the health care provider. Also, provides oversight to DTRs providing care to high risk WIC clients.
Ensures the efficient flow of the nutrition and health assessment and education processes.
Issues NYWIC EBT benefits, Verification of Certification (VOC) transfers to other WIC Programs and seasonal Farmer Market Nutrition check booklets.
Does anthropometric (weighing and measuring) measurements of WIC infants, toddlers, preschool children (to age 5), pregnant / postpartum women and breastfeeding mothers as required by NYS DOH WIC. USDA approved waiver to defer physical presence of WIC applicant / client until further notice to determine nutritional risk.
Determines hematology (Hb: iron level) using HemoCue or Masimo to determine if WIC applicant is anemic. USDA approved waiver to defer bloodwork requirements to determine nutritional risk.
Does related work as needed and assigned.

The Registered Renal Dietitian is an integral part of the interdisciplinary team, using nutrition education to help improve quality of life for our patients. RDs are responsible for assessing, monitoring and educating patients and their families on the importance of diet and nutrition in managing chronic kidney disease.
Schedule: Full-time, 40 hours/week, weekends off
Compensation: Pay range from $34.25-$43.39 per hour, depending on renal experience; experience preferred

Responsibilities
What You Can Expect:
Assist patients with making diet and nutrition choices that improve their quality of life
Complete comprehensive nutrition assessment for all new and at-risk patients
Collaborate with interdisciplinary team and patient to develop and implement individualized care plans based on nutritional assessment; review monthly and document progress toward established goals
Identify specific interventions necessary to meet nutritional needs based on evidence-based guidelines, best practices and current research
Educate patient and family/caregiver about renal diet and fluid management
Monitor monthly laboratory results and weight changes, make recommendations to meet established goals
Utilize a patient-centered care approach, respecting and responding to individual and cultural preferences while encouraging engagement
Participate in Quality Assessment Performance Improvement process, documenting and attending meetings per clinic policy
Compile and create nutrition materials to promote adherence to renal diet guidelines
Communicate nutrition concerns with care team, including attending physicians
Share kidney disease and nutrition advances with care team as needed
Qualifications

Education/Training:
Must be a Registered Dietitian Nutritionist with Commission on Dietetic Registration
Minimum one year professional work experience in clinical nutrition as a registered dietitian (after successful completion of registration exam) required
Previous renal care experience preferred, not required
