Schedule & Compensation

Key Responsibilities
Conduct comprehensive nutrition assessments for patients at nutritional risk, following facility protocols and evidence-based guidelines
Develop, implement, and monitor individualized care plans to support recovery and optimize patient outcomes
Provide nutrition counseling and education to patients and families, focusing on disease management, recovery, and prevention of readmissions
Collaborate with physicians, nurses, therapists, and other members of the care team to deliver coordinated patient-centered care
Ensure documentation is thorough, accurate, and timely within the electronic medical record system
Monitor patient progress, reviewing labs, weights, and clinical status to adjust interventions as needed
Contribute to quality improvement initiatives and serve as a nutrition resource for hospital staff
Qualifications
Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program
Credentialed as a Registered Dietitian (RD/RDN) by the Commission on Dietetic Registration
Current/active state dietitian license (or eligibility)
Minimum of 1-3 years of recent clinical experience in acute or rehab hospital settings (required)
Experience with enteral/parenteral nutrition strongly preferred
Strong communication, critical thinking, and time-management skills
Ability to work independently while collaborating effectively with interdisciplinary teams

Responsibilities
Provide quality nutritional care to patients such as the development, implementation and monitoring of Medical Nutrition Therapies.
work collaboratively with the medical staff, patients, families and food service department to ensure the patients' nutrition care plans are implemented and departmental standards are maintained.
Perform nutritional assessments and re-assessments, identifying goals, recommending nutrition interventions, monitoring and evaluating patient progress.
Develop and monitor enteral and/or parenteral nutrition support regimens.
Work closely with the Speech Therapy Department to assist with the progression of textured modified diets to ensure patient safety and optimize oral intake.
Clarify admission orders to ensure nutrition support, diet and oral supplement orders are consistent with the facility's established nutrition support formulary and diet manual.
Consider religious, cultural and ethnic factors in development of nutrition care plans.
Review yearly updates and approves therapeutic diet manual annually. Educates clinical and food service staff on location of the Nutrition Care Manual. Maintains hard copy of simplified diet manual for downtime.
Keeps current with nutritional practices and theories. Maintains current professional registration requirements (i.e., Professional Portfolio, self-development and continuing education). Demonstrates interest in continuing education, academics or specialized certification to further enhance professional skills.
Evaluate potential food/drug interactions, and/or herb/supplement interactions.
Attend interdisciplinary team meetings and communicating progress and special needs.
Participate in developing, planning and standardizing menus.
Perform meal rounds to ensure patients' meal trays are consistent with the prescribed diet and meet standards as to quality, quantity, temperature and appearance.

Position Summary
The Clinical Registered Dietitian will be responsible for assessing the nutritional status and developing evidence-based nutrition care plans for patients across the lifespan with various acute and chronic conditions. As a member of the Integrated Care Team, the dietitian will work collaboratively with behavioral health consultants, registered nurses, integrated care coordinators, medical assistants, and primary care providers to improve patient health outcomes and organizational goals. The integrated dietitian will provide a stepped care approach to providing concise, action-oriented consults with patients resulting in ongoing support and guidance. The integrated dietitian will also be responsible for establishing Self-Management Goals (SMGs) with their patients and following ups dependent on PCP follow-up visits or patient needs. This position is essential to Adelante's team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.
What You'll Need to Have
· Four (4) year college degree in Dietetics or nutrition-related degree and preferred two (2) years experience in community or clinical nutrition education programs or completion of a dietetic internship with RD/RDN credentials
· Must hold current credentials as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration
· Bilingual (English/Spanish) strongly preferred

What You'll be Doing Day to Day
· Uses the nutrition care process to provide a comprehensive nutrition assessment of the patients nutritional needs, suggests appropriate nutrition therapy interventions, and documents nutrition care plan in the electronic health record
· Provides nutrition education and health promotion to clients and others from the different grants/programs as well as preventative strategies to the Health Education team
· Clearly communicates and coordinates relevant patient information amongst the care team and treating clinicians to maintain proper patient care plans.
· Evaluates and educates patients on the use of nutrition-related medications, dietary supplements, and drug-nutrient interactions
· Develops, writes, and reviews written nutrition related patient education materials as well as educational guidelines for nutrition and preventive health promotion services according to the local standard care plan
· Educates Health Education department staff on nutrition education guidelines
· Refers to appropriate interprofessional team members, resources, or programs as needed
· Assists in the completion of appropriate reports as required
· Conducts quality assurance according to federal guidelines
· Participates in Health Education team meetings, designated Specialty Team meetings, and quality improvement initiatives.
· May develop relationships with external healthcare organizations and facilities in efforts to find additional resources
· Works collaboratively with the Health Education team in data collection, health outcomes reporting, clinical audits, and programmatic evaluation.
· Helps evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.

+ Assesses patient's knowledge of diet and kidney disease and provides education appropriate to patient's learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
+ Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
+ Completes nutritional assessment and participates in the interdisciplinary comprehensive assessments.
+ Participates with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patient's needs, in accordance with established Federal and, where applicable, state licensure guidelines.
+ Calculates diet prescription according to standard of practice.
+ Completes progress notes on all chronic dialysis patients according to established FMCNA guidelines. Maintains complete and accurate documentation of all appropriate information in medical record according to established FMCNA policies and Federal and state regulations.
+ Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining an effective nutritional status.
+ Identifies malnourished patients as well as patients at risk for malnutrition and works collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions. Reviews each patient's albumin level and body weight at least monthly. Implements plan of care with involvement of patient/family/caregiver.
+ Monitors adherence and response to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient's ability to achieve the goals.
+ Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V.
+ Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control.
+ Adheres to FMCNA algorithm and medical record policies.
+ Reports on nutrition QAI results and participates in the interdisciplinary QAI program.
+ Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate.
+ When appropriate, works with Case Manager to coordinate plan of care for patient education and identifies candidates to refer to physician for nutritional supplements, as per disease management agreements.
+ Reviews nutrition related lab results:
- Counsels patient and/or caregiver and formulates appropriate action.
- Recommends treatment changes to the interdisciplinary team as appropriate.
- Communicates with physician and/or facility staff regarding lab results as appropriate.
- Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities (i.e. hospitals, short term and long term care facilities).
+ Participates in pertinent staff meetings (i.e. general staff, quality improvement), Care plan meetings, and others as applicable.
+ Consults with FMS Corporate Dietitian regarding nutrition concerns as needed.
+ Maintains and improves knowledge and skills for a competent and innovative practice.
+ Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable.
+ Collaborates with interdisciplinary team, ensuring that all work areas are safe and clean.
+ Other nutrition management duties as assigned.
