Position Overview:
The Dietitian will be responsible for completing comprehensive nutritional assessments, developing and implementing care plans, and collaborating with an interdisciplinary team to ensure optimal nutritional support for participants. Key Responsibilities:
Participant Care - 60%
Develop and implement individualized nutritional Plans of Care and reassess every six months or as participant status changes.
Screen nutritional status, access to food, and coordinate services such as Meals on Wheels or grocery assistance.
Provide education and dietary counseling to participants and caregivers.
