Responsibilities
* Provide quality nutritional care to patients such as the development, implementation and monitoring of Medical Nutrition Therapies.
* work collaboratively with the medical staff, patients, families and food service department to ensure the patients' nutrition care plans are implemented and departmental standards are maintained.
* Perform nutritional assessments and re-assessments, identifying goals, recommending nutrition interventions, monitoring and evaluating patient progress.
* Develop and monitor enteral and/or parenteral nutrition support regimens.
* Work closely with the Speech Therapy Department to assist with the progression of textured modified diets to ensure patient safety and optimize oral intake.
* Clarify admission orders to ensure nutrition support, diet and oral supplement orders are consistent with the facility's established nutrition support formulary and diet manual.
* Consider religious, cultural and ethnic factors in development of nutrition care plans.
* Review yearly updates and approves therapeutic diet manual annually. Educates clinical and food service staff on location of the Nutrition Care Manual. Maintains hard copy of simplified diet manual for downtime.
* Keeps current with nutritional practices and theories. Maintains current professional registration requirements (i.e., Professional Portfolio, self-development and continuing education). Demonstrates interest in continuing education, academics or specialized certification to further enhance professional skills.
* Evaluate potential food/drug interactions, and/or herb/supplement interactions.
* Attend interdisciplinary team meetings and communicating progress and special needs.
* Participate in developing, planning and standardizing menus.
* Perform meal rounds to ensure patients' meal trays are consistent with the prescribed diet and meet standards as to quality, quantity, temperature and appearance.

Proactively collaborate and communicate effectively with physicians, nurses, therapists, case managers, peer mentors, and other members of the interdisciplinary care team to optimize client outcomes and integrate nutrition seamlessly into the overall treatment plan.
Ensure compliance with all facility policies, state licensing regulations (e.g., specific to Pennsylvania/New Jersey), and best practice guidelines for addiction treatment and nutritional care.
Maintain thorough and timely electronic medical record documentation, reflecting medical necessity and the effectiveness of nutrition interventions in the context of addiction recovery.
Routinely monitor clients for changes in weight, labs, fluid status, and clinical condition (e.g., related to post-acute withdrawal, medication side effects), adjusting nutrition interventions promptly.
Actively participate in quality improvement initiatives, clinical rounds, and case conferences to enhance nutrition care services and overall client well-being within the addiction recovery setting.
Serve as a nutrition expert and resource for facility staff, contributing to staff education on the role of nutrition in addiction recovery and general wellness.
Perform other tasks as assigned and within scope of practice as needed to support client recovery and facility operations.
Experience & Qualifications:
Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program.
Registered by the Commission on Dietetic Registration (RD/RDN).
Current state licensure/certification (if mandated by the state where the facility operates).
Minimum of 2-3 years of recent clinical experience, with a strong preference for experience in behavioral health, addiction treatment, mental health, or a related field. Acute care experience is a plus.
Demonstrated understanding of the nutritional challenges and recovery needs of individuals with substance use disorders, including an awareness of co-occurring mental health conditions.
Proficiency in assessing and addressing nutritional deficiencies, weight management, and specific dietary needs relevant to recovery.

Provide diabetes education and presentations in-person or via webinars or other virtual modes as requested for groups and organizations, including corporate, non-profit, and education systems.
Complete client electronic health records, reporting for ADCES or ADA Recognition, monthly reporting as required by LifeCare Alliance's Wellness department's protocol, as well as client forms that are associated with grants, and any other reports or information required.
With Diabetes Dayton staff, complete annual review, with revision as needed, of the diabetes education service's nutrition and medical curriculum, program forms, and support materials.
Support Diabetes Dayton camps for children with diabetes to ensure dietary needs are being met, appropriate snacks are on hand and educational components are being met.
Provide diabetes education and complete biometrics for staff, interns and volunteers and other organizations and provide workplace wellness education as requested.
Work collaboratively with Executive Director of Diabetes Dayton to help enhance the agency's collaborative relationships with other organizations and community members.

WORKING CONDITIONS:
Work setting will be in office and community locations as necessary. Personal vehicle required for travel. QUALIFICATIONS:

Essential Functions
* Assesses nutritional status of patients based on input from referring physician, medical records, lab results, blood glucose monitoring trends, various clinical assessment methodologies, and patient consultation.
* Assesses nutrition knowledge and self-care behaviors of patients and their support members. Evaluates patients' skills in self-care, ability/readiness to learn, and resource utilization. Collaborates with other members of the interdisciplinary team in gathering data relevant to the patient's health status.
* Collaborates with the patient and the interdisciplinary health care team in developing a patient-centered plan of care. Examines the various treatment options available for implementing the plan of care.
* Designs individualized care plans that are congruent with the preferences, needs, age, educational level, culture, lifestyle, financial resources, physiological status, psychosocial readiness and desired clinical outcomes of the patient.
* Provides Medical Nutrition Therapy and educates clients and support members on the content and strategies to operationalize nutrition goals and assimilate them into long-term behavior changes. Mentors patients and support members in the development and application of clinical problem-solving skills.
* Ensures continuity of care by communicating with patient's health care providers regarding patient needs and ongoing status of care plan.

* Acts as an expert resource and consultant in the field of outpatient nutrition education for physicians, other healthcare professionals, and the community at large.
* Maintains and evaluates patient education materials relating to nutrition using evidence based practice. Develops new materials as necessary. Develops and conducts group presentations and related education programs as needed.
* Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.

Preferred certification registration with American Dietetic Association.

The Toledo Clinic is seeking a part-time Weight Management Registered Dietitian to join our Timberstone Family Practice. This position will be scheduled on Fridays from 9:00am - 2:00pm. The office is located at 4411 Holland-Sylvania Rd in Toledo OH.
The weight management Registered Dietitian supports the primary care physician/ referring provider by providing nutritional assessment, education and management to patients with chronic conditions who are at risk for disease progression and complications. In collaboration with the physician and interdisciplinary team, the Registered Dietician develops and implements an individualized care plan designed to promote the patient's understanding and management of their condition, optimizes quality of life, and improves clinical outcomes. The Registered Dietitian will also help to develop, implement, and assess nutritional programs for the weight management program.
Principal Duties & Responsibilities:

* Conduct comprehensive nutritional assessment, education, and management to patients referred by physicians and/or multidiscipline care team
* Develop, implement, and coordinate individualized meal plans
* Review and analyze patient's nutritional needs and goals to make appropriate dietary recommendations
* Assess patients' knowledge and tailor education to fit patients' needs
* Provide outpatient education and counseling to patients and families in individual or group situations, in accordance with established educational plans
* Collaborate with multidisciplinary team and community professionals to ensure continuity of care
* Communicate regularly with weight management program coordinator
* Maintain timely clinical documentation in compliance with the Toledo Clinic, Inc. policies and regulatory standards
