    How you'll change lives As a Dietitian at U.S. Renal Care, you will be an integral part of an interdisciplinary team, providing excellent care for patients living with kidney disease. U.S. Renal care dietitians work under the supervision of a RDN Regional Manager. What you'll be doing Patient Nutrition. Using the most current, science-based practices, you will recommend therapeutic diets based on patient nutritional needs, preferences, and changes in treatment, in consultation with the patient's physician. You will review lab results and provide appropriate education for nutritional interventions on such topics as fluid and sodium, potassium, and CKD-Mineral and Bone Disorder management. You will also conduct patient assessments and care planning activities, as well as participating in monthly interdisciplinary patient care conferences. Teamwork. You will collaborate with the Medical Director, physician/advanced practice provider, facility administrator, social worker, nurse, and patient care technician to maintain positive relationships with area hospitals, skilled nursing facilities, agencies, vendors, and the community. Safety & Quality. You will help with clinical and operational processes, with a focus on improving patient health and achieving target goals for patient outcomes. You will also help maintain a productive quality assurance program and participate in monthly Quality Assessment and Performance Improvement (QAPI) activities. What we're looking for Current Registered Dietitian (RD/RDN) certification showing proof of registration with Commission on Dietetic Registration (includes Baccalaureate or Advanced degree). Minimum of one-year experience in clinical nutrition as a Registered Dietitian. Current Dietitian (LD/LDN) license in applicable state is required unless employed in the state of AZ, CA, CO, HI, IN, MI, NJ or VA. State certification (Certified Dietitian/Nutritionist) required in CT and NY. License must be maintained as current and in good standing. Demonstrated working knowledge of the English language and ability to communicate verbally and in writing. Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all company clinical applications required within 90 days of hire. Nutrition At Risk (NAR) Meetings : These meetings typically involve the DON, RCMs, the dietary manager, and other relevant members of the care team, such as members of the therapy department. During this meeting, the RD addresses residents with significant weight changes, wounds, nutrition support (such as IV nutrition or tube feeding), dialysis, or any other high-risk needs. The nursing staff updates the team on medication changes, lab values, changes in care plan, changes in food and fluid intake, and other pertinent details. The RD analyzes this information to identify potential nutritional factors contributing to the resident's condition and recommends interventions to address them. These interventions may include adding/reducing supplements, requesting labs, diet changes such as therapeutic or texture-modification, or requesting medication changes. Resident care plans are personalized outlines of resident's goals of care which also may be updated during the NAR meeting, with some facilities assigning this task to the RD or a designated staff member. The care plan is reviewed and updated regularly to reflect the resident's changing condition or progress towards goals. Therapy Staff Collaboration : Dietitians additionally collaborate with therapy staff such as speech language pathologists (SLP) and social workers in several ways to ensure a well-rounded approach to patient care. SLPs play a multifaceted role, evaluating swallowing function, recommending texture modifications and safe swallowing techniques, while also suggesting adaptive equipment like specialized utensils, cups, or feeding assistance to improve mealtime safety and efficiency. Job Overview The Registered Dietitian (RD) - PACE Site is a member of the Interdisciplinary Team (IDT) with both clinical nutrition and food service management roles. The RD will conduct nutrition assessments and follow-ups as needed to assess participants' nutritional status and provide appropriate medical nutrition therapy (MNT), such as therapeutic and mechanically altered diets, oral nutritional supplements or enteral nutrition recommendations, dietary counseling, nutrition education, food security resources, and coordination of care with other IDT disciplines. The RD will support Child and Adult Care Food Program (CACFP) compliance by planning menus to meet requirements, conducting quarterly CACFP food service audits, and maintaining required CACFP documentation. The RD will support meal service by planning therapeutic menus, maintaining diet records, coordinating diet changes with the Food Preparation Coordinator, and conducting regular kitchen audits. The RD will conduct in-services as needed for the kitchen staff and/or IDT members to ensure accurate and evidence-based nutrition care practices. Minimum Requirements Active Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) status with the Commission on Dietetics Registration required. California Food Handler or Food Protection Manager Certification from an ANSI-accredited training provider meeting ASTM International E2659-09 Standard Practice for Certificate Programs. Required within 90 days of hire, required. Minimum of 1 year professional work experience in clinical nutrition as an RD/RDN preferred. Minimum 1 year of experience working with the frail elderly, preferred. Bilingual English/Spanish/Mandarin/Cantonese, depending on demographics of PACE site, preferred. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
