The RD collaborates closely with primary care providers, nursing, community health representatives, and wellness staff to deliver integrated, trauma-informed care. ESSENTIAL QUALIFICATIONS Bachelor's Degree in Nutrition, Dietetics, or related field. Current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian. One year of clinical or community nutrition experience preferred. Experience working with Tribal communities or in rural health settings preferred. DUTIES AND RESPONSIBILITIES include, but are not limited to the following: Conduct comprehensive nutritional assessments and develop individualized care plans for patients referred by providers or through community programs. Provide ongoing education and counseling for patients with chronic conditions such as diabetes, hypertension, obesity, and cardiovascular disease. Deliver nutrition services for maternal-child health, including prenatal nutrition, breastfeeding support, and infant feeding guidance. Support and expand home visiting services, especially for elders and families with limited access to clinic-based care. Develop and implement culturally relevant group nutrition classes and workshops. Lead the nutrition component of the weight management program, including meal planning, behavior change strategies, and tracking progress. Maintain accurate, timely documentation in the electronic health record. Assist with community health initiatives and outreach events promoting nutrition and wellness. Monitor and evaluate program effectiveness and outcomes, adjusting interventions as needed. Maintain current knowledge of best practices in nutrition and dietetics.
