The Renal Dietitian is responsible for providing nutrition education, counseling and interventions as appropriate to End Stage Renal Disease (ESRD) patients and authorized caregivers in clinical settings to maximize the patient's nutritional well-being. What Will You Do? Nutrition Assessment & Care Planning Conducts initial and ongoing nutrition assessments, develops individualized care plans, and updates them in collaboration with the interdisciplinary team (IDT). Medical Nutrition Therapy & Monitoring Designs and adjusts nutrition interventions based on lab results, dialysis adequacy, and patient goals. Monitors progress and documents monthly. Patient Education & Counseling Provides tailored nutrition education using appropriate materials, considering each patient's medical, social, and economic background. Team Collaboration & Facility Support Participates in care conferences and quality improvement meetings. Travels to assigned facilities and provides coverage as needed. Professional Development & Compliance Maintains credentials, stays current with clinical practices, and adheres to CMS regulations and company standards.
