ABOUT THE JOB The Registered Dietitian Nutritionist is part of the integrated health center team, working with the health center providers, staff, and the Population Health and Wellness Team. It supports the primary care setting and expands the comprehensive care of participants. The Registered Dietitian Nutritionist (RDN) provides Medical Nutrition Therapy (MNT) to individual patients and groups, utilizing evidence-based approaches to improve health, prevent disease, and support behavior change. The RD serves as a subject matter expert in nutrition therapy, with a focus on areas such as weight management, diabetes care, cardiovascular health, metabolic syndrome, and the prevention of chronic conditions through nutrition. This role involves planning, organizing, and delivering personalized nutrition care and support for eligible patients, in alignment with client agreements and clinical guidelines. ESSENTIAL DUTIES & RESPONSIBILITIES * Utilize medical nutrition therapy principles to identify dietary improvements to benefit overall health across all stages of the lifecycle and provide patient education for the prevention and management of chronic conditions. * Assess readiness to change, collaborate with patients and health care team to set health related goals, and provide support through individualized nutrition education and motivational interviewing techniques. Provide nutritional guidance based on guidelines and patient's economic, social, and dietary needs and preferences. * Lead a variety of health-related workshops, lunch and learns, cooking demos, grocery store tours and support groups, providing nutrition education. Promote engagement of services through on-site wellness fairs and other outreach strategies, as needed. * Communicate appropriate dietary history and nutrition related goals in the electronic medical record and with other Marathon Health providers while maintaining patient confidentiality at all times. * Provide nutritional consults for Marathon Health care team providers, as needed. * Assist with scheduling medical appointments and making internal referrals, as appropriate. * Other duties as assigned QUALIFICATIONS Bachelor's Degree in Dietetics, Nutrition, or related field at a US regionally accredited university or college and course work accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics and 2+ years of experience working as a clinical registered dietitian. Registered with the Commission on Dietetic Registration (CDR) nationally and licensed as a Dietitian in the state(s) where services are provided. Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V. Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control. Adheres to FMCNA algorithm and medical record policies. Reports on nutrition QAI results and participates in the interdisciplinary QAI program. Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate. When appropriate, works with Case Manager to coordinate plan of care for patient education and identifies candidates to refer to physician for nutritional supplements, as per disease management agreements. Reviews nutrition related lab results: Counsels patient and/or caregiver and formulates appropriate action. Recommends treatment changes to the interdisciplinary team as appropriate. Communicates with physician and/or facility staff regarding lab results as appropriate. Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities (i.e., hospitals, short term and long-term care facilities). Participates in pertinent staff meetings (i.e., general staff, quality improvement), Care plan meetings, and others as applicable. Consults with FMS Corporate Dietitian regarding nutrition concerns as needed. Maintains and improves knowledge and skills for a competent and innovative practice. Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable. Collaborates with interdisciplinary team, ensuring that all work areas are safe and clean. Other nutrition management duties as assigned. Performs other related duties as assigned. Job Summary and Responsibilities As an entry level clinician, the Clinical Dietitian I assesses the nutritional status of the patient using recognized tools of assessment, and recommends and counsels patients, family, and/or caregivers on the appropriate nutrition plan to improve the patient's disease or condition management. Works with general patient population, as well as, with patients in more complex level of care under supervision of a Clinical Dietitian II or Clinical Dietitian Lead. Essential Functions: * Conducts patient screening in accordance with specialized clinical guidelines for those at a specific nutritional risk. Completes a thorough nutrition assessment and diagnosis, considering the clinical condition, varied needs of age-specific populations, as well as cultural, religious and ethnic backgrounds. Uses clinical judgment to identify malnutrition and other nutrition diagnoses. Closely monitors, evaluates and re-screens patient nutrition status as appropriate. Updates plan of care as necessary. Follows up with patient as needed. Promotes a positive patient experience by maintaining communication with patients, staff and guests and providing excellent customer service. * Supports patient interactive positions (i.e. dietitians, diet clerks, etc.) and provides for the operational needs of these positions. Job Requirements Education and Experience: * Bachelor's Degree with completion of accredited dietitian program (ACEND - Accreditation Council for Education in Nutrition Dietetics.) * Experience with specific patient populations (e.g. pediatric, bariatric, ICU, etc.) may be required. Education/Experience: Equivalent to a bachelor's degree in Dietetics or a closely related field approved by the Commission on Dietetic Registration if individual became a dietitian before January 1, 2024.
