Conduct and complete within seventy-two (72) hours of physician's order, nutrition assessment by consulting with patient/youth, evaluating their nutritional needs, and provide nutritional counseling, as needed, when referred by a physician. The Temporary/Relief Registered Dietitian shall acknowledge that patient/youth may include diabetics, renal, hypertensive, HIV positive, and acute/chronic illness;
d. Collect information on patient/youth nutritional history;
e. Participate in committees relating to nutrition, food service, bioethics, performance improvement/quality assurance and other issues;
f. Instruct institutional food service employees on the preparation and distribution of medical diets and provide dietary staff with alternative food product suggestions or allowed/acceptable food substitutions as needed;
g. Provide follow-up consultations with patient/youth as needed or as ordered by a physician;
h. Maintain professional working relationship between Food Services, Correctional Healthcare Services staff; and custodial staff;
i. Comply with and practice universal safety precautions;
j. Maintain security policy at all times;
k. Maintain a safe work environment at all times;
I. Monitor the clinic dietary service program in the Skilled Nursing Facility (SNF), Correctional Treatment Center (CTC), and/or Licensed Hospital to ensure compliance with the California Code of Regulations, Title 22, SNF/CTC Dietary Services requirements and ensures inpatient area dietary menus are current in accordance with regulations, policies and procedures, and;

Role Scope:
We are looking for a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. The Dietitian will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. The Dietitian completes participant nutritional assessments and reassessments, screens and assesses participant's nutritional status and uses pertinent data to plan and implement appropriate nutrition interventions, obtains participant food preferences and makes dietary modifications as needed, in addition to providing nutrition education to participants, caregivers and Habitat PACE staff.
Participant Care
Develops and implements nutritional plan of care for participants that takes into account social circumstances and medical needs, in collaboration with the PCP and interdisciplinary team.

Assesses participants' access to food and makes referrals to interdisciplinary team for meal preparation evaluation and grocery shopping needs.

Provides instruction to participants and caregivers regarding dietary regimens and nutritional requirements (home visits may be necessary).
Develops and implements the nutrition care plans for high-risk participants including medical nutrition therapy recommendations.

Required Qualifications:
Bachelor's degree in food science, nutrition, and/or dietetics

Role Scope:
We are looking for a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. If working in End Stage Renal Disease (ESRD), must have at least one year experience in clinical nutrition as a registered dietitian.
Per the National Agreement, current KP Coalition employees have this experience requirement waived.
Education
Bachelors degree in Food and/or Nutritional Science. License, Certification, Registration National Provider Identifier required at hire Commission on Dietetic Registration Additional Requirements:
Demonstrated knowledge of clinical competencies based on Nutrition Care Standards of the American Dietetic Association, CDA and American Society for Partenteral and Enteral Nutrition.Demonstrated strong communication, interpersonal and customer service skills required.Demonstrated collaboration and problem solving skills required.Demonstrated knowledge of applicable federal, state and local regulations and The Joint Commission requirements.Must be willing to work in a Labor Management Partnership environment.Also refer to Position Specifications outlined in the appropriate collective bargaining agreement.
