Implements plan of care with involvement of patient/family/caregiver. * Monitors adherence and response to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient's ability to achieve the goals. * Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V. * Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control. * Adheres to FMCNA algorithm and medical record policies. * Reports on nutrition QAI results and participates in the interdisciplinary QAI program. * Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate. * When appropriate, works with Case Manager to coordinate plan of care for patient education and identifies candidates to refer to physician for nutritional supplements, as per disease management agreements. * Reviews nutrition related lab results: * Counsels patient and/or caregiver and formulates appropriate action. * Recommends treatment changes to the interdisciplinary team as appropriate. * Communicates with physician and/or facility staff regarding lab results as appropriate. * Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities (i.e. hospitals, short term and long term care facilities). * Participates in pertinent staff meetings (i.e. general staff, quality improvement), Care plan meetings, and others as applicable. * Consults with FMS Corporate Dietitian regarding nutrition concerns as needed. * Maintains and improves knowledge and skills for a competent and innovative practice. * Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable. * Collaborates with interdisciplinary team, ensuring that all work areas are safe and clean. * Other nutrition management duties as assigned. Work Location Buena Vista WIC Clinic - 2315 Buena Vista Street, San Antonio, TX 78207 Work Schedule 7:45 a.m. - 4:30 p.m., Monday - Friday Essential Job Functions * Investigates complaints and/or deficiencies pertaining to dietary and nutrition services subject to Federal, State, and local standards. * Evaluates program and/or service performance to determine compliance with Federal, State, and local guidelines/regulations. * Observes efficiency and effectiveness of programs operations and makes recommendations concerning improvements. * Plans and implements nutrition and dietary services and programs offered by the Department. * Develops and evaluates policies and procedures necessary to carry out specific programs. * Coordinates activities with the staff involved in nutrition services. * Coordinates and plans for effective utilization of this information through training, advising, and educational programs. * Coordinates and evaluates nutrition and dietary services and programs within the Department and with other agencies and institutions; makes recommendations for improvements. * Performs related duties and fulfills responsibilities as required. * Serves as a nutrition expert and resource person for the WIC program and the San Antonio Metropolitan Health Department. * Provides nutrition assessment and counseling for high-risk WIC participants. * Creates a plan of care and provides measurable goals for each high-risk client. * Provides nutrition education training for the WIC staff on a variety of pertinent nutrition topics. * Approves all Local Agency non-contract formula requests. Communicates with the State Agency regarding approval of Local Agency lesson-plans, and all aspects of nutrition education including policies and procedures. Job Requirements * Bachelor's Degree from an accredited college or university with a minimum of 24 semester hours in Food & Nutrition. * Valid Class "C" Texas Driver's License. * Registered Dietitian with the Commission on Dietetic Registration, American Dietetic Association. Preferred Qualifications * One (1) year of experience with the WIC Program or in community nutrition. * Bilingual in English & Spanish. Essential Functions Leadership * Completes annual competency testing and maintains professional growth and development to maintain registration and licensure/certification * Performs job based on applicable federal, state, and local regulations, company policies and procedures and evidenced based clinical guidelines * Performs job within defined scope of practice * Participates in the planning of the department budget * Submits capital budget requests to the Controller/CFO * Participates in developing and implementing policies and procedures related to nutritional care of the patient. * Participates in the hospital's Performance Improvement program by conducting aggregating, trending and acting on data for key clinical nutrition indicators * Annual QAPI department plan is developed with mandatory components * Monthly reports are taken to hospital committees as per facility policy * Monitors quality assurance in the kitchen by ensuring quality measures are routinely done; spot checking logs for accuracy, ensures safe sanitary conditions in the kitchen by completing kitchen safety/sanitation audits and test tray audits per policy. * Participates in the survey process; instructs staff in matters of conduct and disclosure; always maintains a presence while surveyors are on site and directs the timely collection of information required by the survey team * Undertakes corrective action while survey is in progress if appropriate; works with other hospital departments to develop survey plan of correction as needed * Services on various committees as required by company standards, regulations, and/or appointment by immediate supervisor Direct Patient Care * Conducts initial and reassessments on all patients admitted to the hospital * Ensure proper completion of documentation (assessments, care plans, education, discharge planning * Prescribes nutrition therapy to meet estimated needs and monitors tolerance * Provides ongoing reassessment to ensure optimal nutrition therapy is provided * Participates with Interdisciplinary Team meetings to develop, implement, and monitor nutrition care plans * Interviews patients, family members, legal representatives, and/or significant others, and staff to obtain diet history, food preferences, and other pertinent information needed to develop and/or update plan of care * Advocates for patients to protect and promote patients' rights and accommodate individual needs and cultural/religious preferences * Involves the patient/family in planning objectives and goals for the patient * Conducts nutrition education individually and in classes to patients, families, staff, and community as appropriate * Conducts meal rounds to ensure that patients are receiving therapy that meets patient's needs * Coordinates with Culinary Service Manager to resolve patients' concerns or dissatisfactions with diet or meal service Menu Management (if Culinary Services Manager is not a Registered Dietitian, then RD provides the following oversight) * Monitors Culinary Services Manger's oversight of the menu management process * Reviews and approves all patient menus in the facility; As a Registered Dietitian in the CRAFT program, you will empower aircrew with personalized nutritional strategies to enhance endurance, cognition, and recovery.
