EDUCATION/EXPERIENCE A Bachelor's degree in nutrition or related field is required, plus completion of either an American Dietetic Association (ADA)-approved dietetic internship or a program in hospital dietetics approved by the ADA. Must be a Registered Dietitian. Three years of related experience as a clinical dietitian is preferred. Bilingual capabilities (English/Spanish) preferred. LICENSURE Must possess a current license from the Texas State Board of Examiners of Dietitians.

Provides nutritional assessments and information to participants through individual consultations and group nutritional classes. Maintains a good understanding of program and agency safety as well as infection control with OSHA and Joint Commission standards. DUTIES AND RESPONSIBILITIES: Strong collaboration efforts with primary care providers. Documents information in the patient chart implements a plan, provides nutrition counseling, and makes appropriate referrals. Conducts monthly performance assessments in compliance with quality assurance activities as requested. Regular and proficient computer use of our Electronic Health Records (EHR) and systems to ensure adequate care of patients within caseload and communications with the group providers, partners, and administration. Coordinate follow-up schedule with the caseload. New patient scheduling and benefits checking will be provided by CSRs. Consult with patient and family concerning nutrition needs and goals. Input dietary information accurately and in a timely manner (i.e., patient charting, dietary database). Exhibit excellent customer service and a positive attitude toward patients. Follows all agency and program protocols to maintain compliance with OSHA regulations, and Joint Commission. Provide nutrition counseling to program participants. Utilize motivational interviewing counseling techniques during counseling sessions Develop a referral network of community resources to address patient needs. Knowledgeable on the impact of nutrition assessment as a preventable measure for chronic diseases. Perform all other duties assigned by the Director - Chronic Disease Prevention Program in keeping with the philosophy, purpose, goals, and objectives of the Chronic Disease Prevention program and the agency.
