Works with and participates actively in the interdisciplinary team regarding diet and nutritional problems and management, including patient care plans, problem-solving and case review. 3. Provides nutritional education to patients, family, and staff as needed; coordinates use of community resources. 4. Fulfills all requirements, such as reporting, keeping statistical records, making entries into charts and attending meetings as required. Qualifications: 1. Graduate of an approved Dietetic program. 2. Registered by the Commission on Dietetic Registration. 3. 2 years experience as a dietitian focusing on care of the elderly or frail 4.
