Job Duties/Responsibilities
Patient On-boarding & On-going Care:
* Conduct detailed patient consultations, including reviewing health histories,
* and interpreting diagnostic tests.
* Develop personalized treatment plans, including recommendations for exercise, nutrition, lifestyle, and supplementation interventions.
* Educate patients about integrative care programs, treatment options, and preventive care strategies.

Provides quality nutrition services including, but not limited to, screening, initial assessment, re-assessments, nutrition counseling and education, therapeutic diet calculations, and calorie counts. Develops an appropriate nutritional care plan.

Provide professional nutrition services and programs to all Government beneficiaries including active duty military, military family members, DoD civilians, and retirees.

4. Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Provides pertinent information in the medical records, patient plans of care and/or clinical pathways while meeting requirements of regulatory agencies (i.e. Joint Commission, CMS, State and Local Agencies).
