Responsibilities
Job Requirement:
Provides nutrition therapy for patients with diabetes or other nutrition related disorders utilizing laboratory data, nutritional and clinical history as the basis for determining dietary recommendations.
Responsible for conducting nutritional assessments and ongoing follow-up, adjusting diet/nutrition support regimen as
needed, based on changes in the patient's health status.
Ensures appropriate discharge planning in conjunction with patients, families/caregivers and the multidisciplinary team.
Participates as a Multidisciplinary team member of the Pediatric Genetics Metabolic team, attending team meetings
and inpatient rounds as required.
Documents above activities in patient's electronic health record.
Consults and works with community resources, professional organizations and other agencies in implementing care plans.
Assists in developing, updating and revising of patients and families' educational materials. Provides appropriate
education to patients and families.
Participates in division-based quality improvement projects and research.

How you'll contribute:
Dietician approves menus including modified diets, provides nutritional assessments for patients as needed, and provides counsel to patients and their families on dietary plans, food selections, preparation, and nutritional principles. The Dietician monitors the performance improvement plans of the Dietary Services department and provides input in the development and implementation of patient care plans in relation to nutritional needs.
Additional responsibilities include:
Consult with patients and family members concerning dietary needs and goals. May instruct patients and/or families regarding diet therapy programs.
Coordinate the Dietary Services Department with JCAHO standards for food service and assist the Dietary Manager as needed.
Plan modified diets for patients as requested by attending physician/licensed practitioner. Communicate with the Dietary team in a timely manner.
Perform nutritional assessments in a timely manner. Monitor and document patient care on therapeutic diets.
Review patients' charts to determine need for nutritional assessments. Document patient charts with nutritional planning, and update as changes in nutritional needs occur.
