As a RegisteredNurse, you will have the opportunity to:
Provide comprehensive care during patient visits, discuss chief complaints, obtain vital signs, complete health history, and document health information in the EMR system
Conduct phone triage to assess patient needs and coordinate follow-up care

Qualifications:

Conduct on-site surveys of Medicare-Medicaid certified health care facilities to determine compliance with Federal regulations.

Evaluate many aspects of the healthcare facility system from the quality of care to the consumers' rights and physical plant.

Annual Pay Range: $97,958.40 - $124,324.59
Hourly Pay Range: $47.095386 - $59.771439
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS
Clackamas County Health Centers - Behavioral Health Clinics is seeking a full-time Psychiatric Nurse to provide triage, evaluations, and ongoing care for individuals experiencing mental health and substance use disorders. This position primarily serves Medicaid, Medicare, and medically uninsured populations. The Psychiatric Nurse works collaboratively with Psychiatrists and Psychiatric Nurse Practitioners to support psychiatric treatment plans, conduct evaluations, and assist with medication management. This role requires registration as a Qualified Mental Health Professional (QMHP) in Oregon due to the behavioral health care setting.
Key responsibilities include assessing and preparing clients for prescriber appointments, coordinating care to ensure efficient patient flow, and supporting an integrated treatment team. The Psychiatric Nurse exercises independent clinical judgment, demonstrates effective and respectful communication, conducts timely and accurate evaluations, and maintains comprehensive documentation within the EPIC electronic health record system. This position provides services to both children and adults and involves delivering nursing care, education, and support in a fast-paced clinical environment.

Required Minimum Qualifications / Transferrable Skills:
* Must possess an active, unencumbered RegisteredNurse (RN) license issued by the State of Oregon.
* Must meet the Oregon Administrative Rules (OAR) criteria for Qualified Mental Health Professional (QMHP) and hold current QMHP registration.
* Basic Life Support (BLS) certificate.
* Master's degree in Nursing.
* Strong communication and interpersonal skills, with the ability to establish effective relationships with patients, families, and colleagues.
* Ability to work effectively in a dynamic, fast-paced environment; skilled in prioritizing tasks and managing time efficiently.
Preferred Special Qualifications / Transferrable Skills:
* Bilingual preferred, but not required.
* Experience with Nursing Care Management for patients with chronic conditions.
* Experience with EPIC (Electronic Health Record).
* Experience working in Mental Health settings.

TYPICAL TASKS
Clinical Care and Patient Support
* Conduct comprehensive nursing assessments and evaluations of patients receiving psychiatric and behavioral health services.
* Collaborate with Psychiatric Nurse Practitioners to develop, implement, and monitor individualized treatment and medication plans.
* Provide patient education on medication management, recovery goals, and health maintenance as part of overall care coordination.
* Monitor patient progress, document observations, and adjust care plans as needed to support optimal outcomes.
Mental Health and QMHP Functions
* Function in a Qualified Mental Health Professional (QMHP) capacity to evaluate, assess, and coordinate care for patients within behavioral health programs.
* Conduct biopsychosocial and risk assessments; assist in developing and monitoring safety and crisis plans.
* Provide therapeutic interventions, education, and referrals in collaboration with behavioral health teams.
* Maintain QMHP registration and participate in required competency and continuing education training.
Care Coordination and Collaboration
* Participate in interdisciplinary team meetings to ensure coordinated, integrated, and patient-centered care.
* Communicate and collaborate effectively with physicians, clinicians, and external providers to facilitate treatment and care transitions.
* Serve as a liaison between patients, families, and community partners to address barriers to care, promote engagement, and support recovery.
Documentation and Compliance
* Maintain accurate and timely clinical documentation in accordance with federal, state, and county regulations.
* Utilize the electronic health record (EPIC) to record and track patient encounters, assessments, and care plans.
* Ensure compliance with HIPAA, OAR, and other applicable behavioral health and nursing practice standards.
WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday. Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregonor Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
