ABOUT THE JOB: Family Support Services provides personal nurse visits that include comprehensive maternal and infant assessments, case management, linkage to community resources, and parent education and skills building. Programs include CaCoon (case management for children with special health care needs), Babies First! (prenatal, post-partum and children 0-5 home visiting), and Family Connects (universally-offered new baby home visiting). These are nurse home visiting and case management programs through Oregon Health Authority.
