About the position:

A Brief Overview
Functions as a member of the interdisciplinary team. Primary responsibility to care for patients with behavioral health needs. (Individualized refers to the patient's developmental, cognitive, cultural, psychosocial, spiritual, and educational/family influences and physical/medical care needs.)
Admission/Initial Assessment:
Collects and documents data related to patient's individualized care.
Demonstrates age-specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patient needs, knowledge of growth and development, and understanding of the range of treatment needed by these patients.
Assesses patient/family need for education and uses an individualized approach in delivery style.
Assesses family expectations for, and involvement in the assessment and continuous care or treatment of the patient.
Identifies nursing diagnoses, collaborative problems, and/or protocols for each patient, based on assessment, at the time of admission.
Shift Assessment/Reassessment
Performs reassessment based on unit standards, CPG's/ND's, protocols and changes in patient condition. Communicates changes to appropriate persons (MD, Charge RN, family) as indicated.

Demonstrates ability to prioritize patient interventions based on assessments/reassessments

Reviews and updates computerized care plan to accurately reflect the plan of care.

You will perform Apheresis procedures autonomously in multiple settings, including critical care, emergency room, and freestanding outpatient centers.
Perform venipunctures, access central venous catheters, and access vortex and bard ports.
Perform all phases of therapeutic apheresis procedures (plasma exchange, red cell exchange, platelet, and white cell reduction, photopheresis, and hematopoietic progenitor cell collection), to ensure safe and appropriate care of the patient and explain, as needed, the procedures for patients and their family and other attending healthcare professionals.
Act as a consultant to external health care personnel about clinical apheresis.
Assess patient status relative to therapeutic apheresis, prior to and during procedures, utilizing the nursing process, laboratory testing results, and other clinically significant procedures.
Coordinate closely with physicians and nurses at a variety of hospitals to arrange therapeutic apheresis treatments and facilitate coordinated care.
Evaluate patient medication for compatibility and safety of apheresis procedure and determine when to contact the physician.
Assemble, restock, QC, and maintain supplies and equipment in a state of readiness to maintain a clean, safe, and efficient work environment.
The American Red Cross will provide all necessary training. No experience is required.

About the position:
The RegisteredNurse (RN) Case Manager coordinates all aspects of the patient's home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient's eligibility for home care services & develop the plan of care to be followed.
Qualifications:
Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
Current state license as a RegisteredNurse.
Current state Driver's License.
One-year experience as a RegisteredNurse, two preferred.
Proof of current CPR and Hepatitis consent/declination.
Reliable transportation with valid and current auto liability insurance.
