Assess, plan, coordinate, implement and evaluate care for eligible members with chronic and complex health care, social service and custodial needs in a nursing facility or home and community-based care setting.
Coordinate care across the continuum of services and assist members' physical, behavioural, long-term services and supports (LTSS), social, and psychosocial needs in the safest, least restrictive way possible while considering the most cost-effective way to address those needs.
Facilitate authorisation, coordination, continuity and appropriateness of care and services in the community or HCBS.

Develop a plan of care in conjunction with members or caregivers to identify services to meet the member's specific needs, and goals.
Identify resources needed for a fully integrated care coordination approach including facilitating referrals to special programs such as Disease/Chronic Condition Management, Behavioral Health, and Complex Case Management.
Collaborate with the member's health care and service delivery team including the DSHP Plus LTSS Member Advocate, ICT, and discharge planners, to coordinate the care needs and community resources for the member in order to maintain the member in the least restrictive safe environment possible. Assist members in developing, implementing and amending a back-up plan for gaps in provider coverage.
Ensure approved support services are being provided as outlined in the plan of care. Evaluate the effectiveness of the service plan and making appropriate revisions as needed in accordance with per policy & procedures and state contractual requirements.
Assist members in overcoming obstacles to optimal care through connection with community resources, including communicating with providers and formulating an appropriate action plan.
Document all case management services and intervention in the electronic health record. Adhere to all company, State and Federal requirements related to privacy practices, HIPAA, and quality performance standards.

Qualifications:
RegisteredNurse in the state of DE
5 years of Intensive Case Management and Discharge Planning experience
Experience in completing Assessments, developing Service Plans and Care Plans
Experience collaborating with PCPs, Occupational Therapists, Behavioural Health, and Providers
Experience with ordering DME Equipment
Experience in educating and providing resources for the members' Social Determinants. They must have experience with discharging members from a Facility setting.
Working flexible hours to meet members' needs

The Surveyor will serve as a team member or team leader on various types of surveys (i.e. re-certification, comparative, complaint investigation, and revisits) for long-term care and non-long-term care surveys, which can include; ambulatory surgical centers (ASC), Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICFs/IID), end-stage renal disease (ESRD), Psychiatric Residential Treatment Facility (PRTF), hospital, critical access hospital, and hospice facilities. Surveyors travel to healthcare facilities nationally conducting surveys to assess compliance with requirements and regulations guiding the quality of care for residents/customers of the facilities. The responsibility of the Surveyor is to apply approved survey protocols for conducting on-site surveys. Specific activities include the following:
Conduct on-site surveys of Medicare-Medicaid certified health care facilities to determine compliance with Federal regulations.
Surveys require preparations such as off-site preparation, tour of the facility, observation of care, record review, interviews of staff and families, etc. Determine if care, treatment, and services are consistent with Federal requirements.
Evaluate many aspects of the healthcare facility system from the quality of care to the consumers' rights and physical plant.
Communicate the non-compliance issues with key personnel throughout the site review and during the exit conference to assure an understanding of the deficiencies cited.
Determine if a plan of correction is acceptable.
Conduct follow-up visits and monitoring surveys to ascertain if facilities' plans of correction to resolve the deficient practice have been implemented.
Training: Assist with facility and bureau training. Participate in work groups as needed

The Director of the Interventional Congenital Cardiac Catheterization Laboratory ("The Director") will oversee the catheterization program's medical direction in collaboration with other divisions and programs of the Cardiac Center, including cardiovascular surgery, cardiac anesthesia, nursing, and hospital administration. The Director will lead the Cardiac Catheterization team of pediatric and congenital interventionalists, nursing and technicians in performing diagnostic and interventional congenital catheterization procedures at our hospital.
Congenital and Interventional Catheterization Program Highlights and Clinical Responsibilities
The Director will be one of two full-time pediatric interventional cardiologists who perform the full complement of catheterizations from newborn through adults with congenital heart disease and provide 24/7 cath coverage at our hospital. We currently perform approximately 300 diagnostic and interventional cath procedures annually, including post-transplant surveillance evaluations (biopsies) and hybrid interventions. We are enrolled in the IMPACT and C3PO registries. Additional registry enrollments are pending.
The Director will work with hospital administration and the Cardiac Center leadership team to optimize the program's structure and administrative organization.
The Director will be responsible for ensuring excellent procedural outcomes and for providing cutting-edge interventional technologies in the cardiac catheterization laboratory.
The Director will lead and encourage all academic aspects of the program and its members, including scientific investigation, implementation of robust quality improvement processes, and Fellow education/curriculum development.
The Director will oversee data and inventory management for the program's clinical activities.
Additional Clinical Responsibilities
The Director will care for patients and their families both in inpatient and outpatient settings. Night and weekend call coverage will be performed in rotation with other members of the cardiology team.
The Director will participate in Cardiac Center conferences including but not limited to surgical and cath conferences, Morbidity & Mortality conferences, etc.

Position Details:
Reports to: Nursing Supervisor
Schedule: Part-Time, Night shift schedule (two 12-hour shifts per week)
Location: Milford, DE (On-site)
Key Responsibilities:
Conduct comprehensive nursing assessments, including ASAM forms.
Provide routine care at various levels of care.
Identify treatment needs, integrating findings into collaborative, measurable treatment plans.
Complete nursing documentation in accordance with facility and state requirements.
Review medication logs and assist the medical department in a variety of tasks.
Administer medications as directed by the medical provider.
Facilitate medication education and incorporate nursing and medical issues into treatment plans.
Collaborate with patients, families, and referral sources to provide progress updates and coordinate post-discharge recovery plans.
Apply clinical judgment and intervention skills in crisis situations, including risk assessments and de-escalation.
Tailor nursing approaches to meet diverse patient needs.
Build rapport and foster trust with patients through strong interpersonal skills.
Required Qualifications:
Active RN or LPN license in Delaware.
Nursing degree/diploma from an accredited school of Nursing.

**Job Responsibilities** (listed in order of importance and/or time spent)
+ Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals. Participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
+ Assesses patient needs and physical status at each visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order.
+ Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals.
+ Effectively and timely communicates with Naven Health's Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Naven Health's policy.
+ Under close supervision, coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care.
+ Demonstrates compliance with agency operations, Naven Health's policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards.
+ Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.
+ Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary.
+ Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Naven Health's mission, values, and objectives.
