RegisteredNurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Performs the duties of Home Care Coordinator on the IDT as needed / assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed.
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.
Performs other duties as required or requested.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
**NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (must have within 90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and / or managed care preferred.
1 year experience providing care as an RN required.
Bilingual with Fluency in Hmong required.
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Purpose: A patient navigator is defined as a medical professional whose clinical expertise and training guides patients and their caregivers to make informed decisions, collaborating with a multidisciplinary team to allow for timely cancer screening, diagnosis, treatment, and increased supportive care across the cancer continuum. In essence, nurse navigators guide patients through the healthcare system and help them to overcome barriers to optimal care. The navigation of patients with cancer requires extensive knowledge, specialized skills, and good clinical judgment, which can improve outcomes. This position will work in coordination with St. Agnes Medical Center and cCARE.
Imaging Center Duties
* At the time of diagnostic abnormality is found at St. Agnes Imaging, Cancer Care Nurse coordinator would be notified. (currently a list is provided) At that time, the coordinator would contact primary care and inform them of what has been found and the multidisciplinary program we have to offer.
* The care navigator would contact the patient and arrange for a breast consult. Ideally this visit would be prior to the biopsy.
* Navigator would help to follow-up on the biopsies. If the patient has not been seen by the breast surgeon, results are given back to the primary physician, and again the nurse navigator would attempt to get the patient scheduled in consultation with the breast surgeons.
* They would do an initial intake, including a genetic screen tool to look for patients meeting NCCN guidelines for testing. If patient meets criteria, MD's informed and testing ordered at initial visit. Consults placed to genetics.
* This person will help to be the bridge between St. Agnes imaging and the breast center
cCare Duties
* If the biopsies results indicate a malignancy, the nurse navigator would help to schedule appointments with the multidisciplinary team ( med onc, rad onc)
* Nurse Coordinator will then help to oversee studies that need to get done, and additional visits that are required. She will help coordinate visits and testing on the same day to limit amount of travel, particularly for patients that need to travel a distance.
* Nurse Coordinator would be the point person for the newly diagnosed breast cancer patient to call with questions on appointments, test schedule, etc.
* Help to initiate consults to social work, nutrition, genetics as needed.
Experience, Qualifications/Education:
* High school diploma or equivalent required
* Must have current RN license with the California State Board of Nursing
* Current CPR certification required
* Previous Breast/Patient Navigator experience preferred
* Strong customer service background, preferably in healthcare environment
* Excellent verbal communication skills
* Knowledge of drug names, both brand and generic, and available dosage forms
* Ability to manage and prioritize multiple tasks
* Proficiency in computer processing functions and other standard office equipment
* Ability to calmly and professionally resolve customer issues with diplomacy and tact

PRIMARY RESPONSIBILITIES

Perform comprehensive nursing assessments including physical, emotional, and psychosocial evaluations.
Assess, manage, and alleviate symptoms such as pain, shortness of breath, agitation, nausea, anxiety, and other distressing conditions.
Implement and adjust the plan of care in collaboration with the hospice physician and interdisciplinary team to meet changing patient needs.
Administer medications and perform prescribed treatments according to hospice protocols and physician orders.
Conduct medication reconciliation at each visit to ensure safety, accuracy, and appropriateness.
Provide education and guidance to families and caregivers on symptom management, medication administration, and end-of-life comfort measures.
Offer emotional and spiritual support to patients and families, maintaining sensitivity to cultural and personal values.
Coordinate with the interdisciplinary team (IDG), including social workers, chaplains, CNAs, and volunteers, to ensure seamless and holistic care.
Report and communicate changes in condition promptly to the hospice physician and leadership team.
Participate in on-call triage, facility coordination, and home visits as needed to maintain continuous care coverage.
Ensure that all clinical notes, assessments, interventions, medication updates, and communications are accurately documented in the EMR within 24 hours of the patient encounter.

QUALIFICATIONS
Current RegisteredNurse (RN) license in the State of California (required).
Minimum 1 year of hospice or palliative care experience (preferred).

Additional Duties
Participating in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training.
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises and evaluates home health aide care provided every two (2) weeks.
